Abstract

   Peristiwa kehamilan bagi seorang perempuan serta keluarganya penuh dengan ketidakpastian, diantara ketidakpastian tersebut adalah tanggal persalinan. Hal ini menjadi sumber kecemasan apabila kehamilan telah melewati taksiran persalinan yang berdampak terhadap keputusan untuk mengakhiri kehamilan melalui tindakan induksi atau operasi. Hubungan seksual pada akhir kehamilan diketahui dapat memicu terjadinya persalinan. Studi kasus ini bertujuan untuk mendeskripsikan asuhan kebidanan pada ibu hamil yang melewati taksiran persalinan dengan diberikan anjuran untuk melakukan hubungan seksual. Metode asuhan dilakukan dengan tujuh langkah manajemen kebidanan yang diberikan secara komprehensif mulai trimester ke III kehamilan sampai dengan masa nifas. Hasil Analisis asuhan yang dilakukan mulai trimester III kehamilan tidak ditemukan kondisi patologi, usia kehamilan aterm namun timbul kecemasan karena tanda persalinan belum muncul setelah lewat waktu tanggal taksiran. Anjuran yang diberikan adalah melakukan hubungan seksual untuk merangsang inisiasi persalinan. Tanda persalinan muncul 2 hari setelah asuhan terakhir dan kehamilan tidak lewat waktu. Outcome bayi baik, tanpa adanya kelainan. Asuhan komprehensif dan berkelanjutan dengan memberikan solusi non farmakologis untuk menstimulasi persalinan pada kasus ini dapat mengatasi masalah yang terjadi pada klien.

Abstract

Pilihan tempat bersalin merupakan hak bagi setiap perempuan untuk pengalaman persalinan yang positif. Melahirkan di rumah merupakan pilihan bersalin yang sah dan aman sebagai upaya meningkatkan kepercayaan perempuan terhadap bidan dengan memberikan dukungan secara konsisten melalui asuhan komprehensif. Pada kasus ini, seorang ibu dengan kehamilan ke empat, ingin bersalin dirumah sesuai kepercayaan dan tradisinya. Asuhan yang berkelanjutan diberikan mulai pada trimester ketiga, advokasi untuk bersalin di fasyankes telah dilakukan namun tidak dapat mengubah keinginannya bersalin di rumah. Pertolongan persalinan dirumah dilakukan dengan berbagai persiapan diantaranya: pendamping persalinan, peralatan, persiapan kebutuhan dasar ibu dan bidan, serta persiapan kegawatdarurata. Saat bersalin terjadi  atonia uteri dan dapat diatasi dengan melakukan manemen atonia uteri. Bendungan ASI yang terjadi pada masa nifas dapat teratasi dengan tindakan perawatan payudara. Tempat bersalin merupakan pilihan setiap wanita, bidan harus menghormati pilihan klien dengan memberikan advokasi pilihan melahirkan. Wanita yang  ingin bersalin di   rumah  harus diberi informasi tentang potensi risiko komplikasi atau kegawatdaruratan yang dapat terjadi pada janin atau ibu secara tiba-tiba yang dapat meningkatkan morbiditas bahkan mortalitas pada ibu atau bayi baru lahir. Persalinan di rumah yang terencana dengan baik adalah satu-satunya pilihan ketika perempuan dan keluarganya bersikeras untuk melakukan persalinan di rumah dan setiap bidan harus bersiap menghadapi setiap kondisi darurat  dan memberikan pertolongan persalinan yang aman di mana pun.

Abstract

Background: Breastfeeding is a nutritional need for babies during growth and development. Breast milk (ASI) helps in sensory, cognitive, and motor development and protects and prevents babies from various diseases and infections. Breast milk production is influenced by the hormones prolactin and oxytocin, food intake, the mother's psychological condition, breast care, and the frequency of breastfeeding. In stimulating the release of these hormones, one of the efforts made is breast care. Breast care will stimulate the breasts and affect the hypopise to release more progesterone, estrogen, and oxytocin hormones. The hormone oxytocin will contract other cells around the alveoli, causing milk to flow down towards the nipple. One of the breast care techniques is oketani massage. This massage is a massage technique popularized in Japan that will make the breasts softer, the areola and nipples elastic, making it easier for babies to suckle breast milk. Objective: To determine the effect of oketani massage on postpartum maternal breast milk production. Method: This journal search uses keywords, Boolean Operators and E-data based with literature review and experimental research types. Results: Based on this study, it was found that there was an effect of Oketani massage on breast milk production in postpartum mothers. Conclusion: It is expected that health workers can use the Oketani massage technique to help breast milk production in postpartum mothers as an easy and inexpensive alternative that can help mothers in breast milk production.

Abstract

Perineal wounds are often causes pain and discomfort in patients, thereby reducing mobilization and inhibiting the wound healing process. Cold compresses have a local analgesic effect, reducing nerve sensitivity and lowering the pain threshold and reducing swelling. The aim of this research is to apply cold compresses as a method to reduce stitching wound pain during postpartum care.  This research method involves searching for articles using e-data search engines based on Pubmed, Science Direct, and Google Scholar for 2013-2023 with the criteria of original articles with experimental methods. From the search results, 3 articles were obtained that met the criteria. Applying cold compresses during the postpartum period as a non-pharmacological intervention can be an alternative to reduce pain in perineal wounds.

Abstract

Background: Exclusive breast milk is pure breast milk without additional food or additional fluids such as formula milk, water, etc. which is given to newborn babies before the baby is 6 months old. Woolwich massage has several benefits, including increasing prolactin and oxytocin reflexes (let down reflex), preventing blockages, increasing breast milk production and preventing inflammation or breast dams.. Objective: To determine out that giving Woolwich massage can lead to smooth breast milk production for postpartum mothers. Method: The method for writing this EBCR is by searching for evidence through data based Pubmed and Google Scholar with the publication year 2018-2023, 4 articles were obtained for critical review. Results: After evaluation before and after the Woolwich massage, the mother's breast milk production became smooth.. Conclusion: Giving Woolwich massage is effective in helping to facilitate breast milk production.

Abstract

Pregnancy triggers complex metabolic changes, particularly in glucose homeostasis. This vital adaptation, marked by physiological insulin resistance in the second and third trimesters, ensures an adequate glucose supply for the fetus. However, a failure of pancreatic beta-cell compensation can lead to Gestational Diabetes Mellitus (GDM), a significant obstetric complication. GDM shares pathophysiological similarities with type 2 diabetes (T2D) and poses substantial risks, including adverse perinatal outcomes (e.g., macrosomia and preeclampsia) and long-term consequences such as increased maternal T2D risk and offspring susceptibility to obesity via fetal programming. Despite these known impacts, GDM diagnosis remains challenging due to a lack of uniform diagnostic standards and often-delayed screening. This review, therefore, aims to comprehensively examine the physiological adaptations, factors influencing glucose dysregulation, and clinical implications of GDM. Furthermore, it evaluates current management strategies and identifies future research directions, emphasizing the crucial role of midwives in developing innovative, prevention-based approaches from preconception through the breastfeeding period to improve overall maternal and child health.
