Abstract

This research uses the Root Cause Analysis (RCA) method to analyze the main causes of defects in bulk container bag/jumbo bag products at PT The four main types of defects, namely hole body, off-center printing, folded body, and dirt, were identified using the Pareto diagram, with hole body being the highest category (37%). The P-chart shows that the production process is within statistical control limits. Fishbone analysis found five main factors that cause defects: people, materials, machines, methods and work environment. Suggested solutions include employee training, stricter raw material selection, machine maintenance, work environment improvements, and implementation of standard operating procedures. The results of this research provide recommendations for quality improvement to reduce defect levels.

Abstract

This research analyzes differences in daily flow and water level using Two-Way ANOVA in the context of ISO 9001:2015. The study was conducted to evaluate significant differences in data during January-March and their impact on continuous improvement. Flow and water level data showed a normal distribution, reflecting stability, but the homogeneity test results revealed significant variations. Tukey's post hoc test showed significant differences between January-February and February-March, but not between January-March. These variations indicate the need to evaluate causal factors, such as weather patterns or operational instability. The research recommends standardizing procedures to maintain consistency of operational processes. Improved monitoring systems are also needed to ensure data reliability. Regular training for staff is an additional solution to reduce variability and support quality. This step is expected to strengthen the implementation of the principles of ISO 9001:2015.

Abstract

PT Petrokimia Gresik is a subsidiary of PT Pupuk Indonesia which is engaged in the fertilizer producer sector. This study was conducted to analyze the level of service satisfaction, measure whether there is a significant difference before and after implementation using the normality test, non-parametric statistics (sign test and Wilcoxon test), and correlation test. In the sign test and Wilcoxon test, 0 data with negative differences, 0 data ties, and 100 data positive differences were obtained from a total of 100 data. In the sign test, the Z-score value was -9.900 and in the Wilcoxon test, the Z-score value was -8.815. In the normality test, the significance value is 0.000, and 0.000 where ? 0.05, indicating that H0 is rejected, then the data is not normally distributed, in the sign test, the significance value is 0.000 ? 0.05 which means H0 is rejected, then there is a significant difference between before and after the implementation of Smart Warehouse Self Check-In and Check-Out, in the Wilcoxon test, the significance value is 0.000 ? 0.05 which means H0 is rejected, then there is a significant difference between before and after the implementation of Smart Warehouse Self Check-In and Check-Out, in the correlation test, the significance value is 0.000 ? 0.05, then H0 is rejected and H1 is accepted, meaning there is a correlation between the data before and after the implementation of Smart Warehouse Self Check-In and Check-Out. This research can help solve problems and improve the level of service at the Warehouse and Packing Department counters.

Abstract

Occupational health and safety (OHS) in the phosphate acid plant industry is crucial due to the potential hazards from chemical substances and mechanical equipment. This study analyzes hazard identification, risk assessment, and control measures using the HIRADC method at PT XYZ. The results show that HIRADC effectively reduces risks from high to moderate or low categories. Key identified hazards include liquid spills, forklift-related risks, conveyor entrapment, ball mill noise, and phosphate rock dust exposure. Control measures implemented include regular cleaning, safety signage, personal protective equipment (PPE), and the LOTO system. This study recommends further research to enhance risk management effectiveness.

Abstract

PT XYZ is a manufacturing company engaged in the plastic container packaging and printing industry, with a focus on high product quality. To maintain production quality, the company has a Quality Control (QC) department that is responsible for controlling and ensuring that the products produced meet the standards. However, the company often faces complaints from customers regarding product inconsistencies, such as inappropriate colors, flowlines, unmolded products  and black spots. Based on the analysis, it was found that the body travel square product has the  highest reject rate  , with a total of 2015 pcs rejects. The types of non-conformities that occurred included black spots (1047 pcs), non-standard colors (624 pcs), unmold (180 pcs), and flowline (164 pcs). To overcome this problem, analysis was carried out using Six Sigma method and Kaizen analysis, as well as Statistical Quality Control. The results of the analysis show that the average Six Sigma value is 3.22 with a minimum target of 3.4 or equivalent to 6 sigma and the average DPMO value is 66,126, indicating that the production quality is quite good.

Abstract

The problem of urban waste in Indonesia is a problem that has not been completely solved, it also occurs in the Tanjung Perak Port area. This problem can be overcome by regulating the process of moving and transporting waste based on optimal routes. The purpose of this study is to analyze the waste transportation system in several Depots (TPS) in the Tanjung Perak Port area and compare the transportation cost needs in each waste transportation pattern to find the most optimal route. The stages carried out are to collect data on the distance of the Pool to each Depo/TPS, the distance between TPS, and the distance of each TPS to the Benowo Landfill. The application of the Dynamic programming Algorithm is used to determine the shortest distance, and calculate the minimum operational cost. The results of the study show that the Dynamic programming algorithm can determine the optimal route for the problem of waste transportation routes at each TPS at Tanjung Perak Port with the number of Dump trucks as many as 1 vehicle and the calculation of fuel cost needs per day of Rp.88,413,-.
