Abstract

HIGHLIGHTS1. Clinical gynecology examinations must adapt to pandemic situation.2. It must pay attention to universal precautions, aseptic and sterile technique to minimize infection transmission.3. In terms of reducing contact, the need for examination are divided by priority into "now", "soon", or "later".4. Gynecology examinations can be carried out with direct examinations by medical personnel wearing proper personal protective equipment if needed.5. Patient counseling can be done by remote consultation or telemedicine. ABSTRACTCOVID-19 is a recent pandemic caused by the SARS COV-2 agent with a high incidence and mortality. The disease is transmitted through respiratory droplets and direct contact. Clinically this COVID-19 patient is mainly related to the respiratory tract symptoms. The current clinical classifications are divided into suspected, probable, and confirmed cases. To reduce transmission must pay attention to universal and hierarchical precaution, aseptic standards, and sterile techniques. The types of gynecological examinations during a pandemic are the same as those in general, except that the methods, settings, and priorities are different. The examination begins with screening to assess the risk of transmission so that it can determine the place of examination. The urgency of the examination, history of TOCC, local transmission, provider, and room conditions also need to be considered. Counseling during a pandemic can be done in person or by telemedicine. Counseling is provided for general and case-specific gynecological information. Each gynecological case requires a different focus on counseling.

Abstract

HIGHLIGHTS 1. One of the most prevalent disease in females worldwide is cervical cancer.2. Simple and safe modality with high efficacy to treat cervical precancerous lesions before being progressed to cancer is necessary. 3. Trichloroacetic acid (TCA) has those advantages, so it will become an effective treatment for cervical precancerous lesions in the future.   ABSTRACT Cervical cancer becomes one of the most prevalent disease in female worldwide. Human papillomavirus (HPV) is main etiology of cervical cancer, thus this disease is preventable. Before progressed into invasive cervical cancer, cervical precancerous lesions developed and classified into 3 stages: CIN1 (LSIL), CIN2, and CIN3 (CIN2+ also referred as HSIL). World Health Organization (WHO) arranged ‘screen-and-treat’ programme to treat cervical precancerous lesions immediately before it progressed to cancer. However, a simple and safe modality with high efficacy is necessary to accommodate this strategy. Trichloroacetic acid (TCA) has those advantages and some research suggested high efficacy to treat cervical precancerous lesions with simple, safe, and cost-effective. TCA has potential to become effective treatment for cervical precancerous lesions in the future.

Abstract

HIGHLIGHTS 1. Preeclampsia, or hypertension in pregnancy, remains a serious problem that has negative short and long-term consequences for the mother and the baby.2. The use of antihypertensive medications has its own precautions as it they may harm the fetus.3. Prophylactic measures with minimal or moderate impact may be beneficial for mothers with hypertension in pregnancy.   ABSTRACT Treatment of hypertension in pregnancy, such as preeclampsia (PE), is still a difficult issue with negative short and long-term consequences for both the mother and the baby. Screening for preeclampsia at 11-13 weeks' gestation using a combination of maternal demographic characteristics and medical history with biomarker measurements can identify approximately 75% of women who develop premature preeclampsia with delivery at 37 weeks gestation and 90% of those with early preeclampsia. Preeclampsia has a 10% positive screen rate at 32 weeks. Another important worry on the use of antihypertensive medications during pregnancy is the potential harm to the fetus. Methyldopa, hydralazine, labetalol, and nifedipine are some common antihypertensive medications. Aspirin use is frequently related to a decrease in the prevention of early preeclampsia, but it must be accompanied by medication adherence. Aspirin can be coupled with heparin. Recent investigations on the use of furosemide and nifedipine in preeclampsia have also revealed a new combination.

Abstract

HIGHLIGHTS Clinicopathologic characteristic of patient with LEEP procedures dominated by women above 35 years old, with total parity less than 3 and without history of contraceptive usage. The p16 expression analyzed by immunohistochemistry technique. The positive and negative reactivity of p16 ratio was 3:5 on LEEP specimen which microscopically appear as chronic cervicitis, low-grade and high-grade squamous intraepithelial lesion.   ABSTRACT Objective: The aim of this study was to reveal p16 expression with the clinicopathological characteristics of patients who underwent cervical biopsy using the Loop electrosurgical excision procedure (LEEP) at Udayana University Hospital, Bali, Indonesia, for the period 2020 – 2023. Materials and Methods: The research was conducted at Anatomic Pathology Laboratory of Udayana University Hospital, Denpasar, Bali, Indonesia. The samples were selected based on inclusion criteria such as the formalin fixed paraffin embedded (FFPE) sample from positive IVA patient and continue for LEEP procedure. Otherwise, the exclusion criteria were moldy FFPE sample and incomplete clinical data. Then, p16 immunostaining procedure was carried out manually. The interpretation of p16 results was analyzed using SPSS software, version 25 by International Business Machines (IBM) Corporation. Results: The positive p16 expression was revealed in 12 samples (38.7%), in contrast the negative staining appeared in 19 samples (61.3%). Unfortunately, p16 expression was not significant statistically based on age, parity, and contraceptive history, with p-values of 1.00, 0.45, and 0.65, respectively. Meanwhile, a statistically significant association was found between p16 expression and histopathologic diagnosis (p = 0.02, 95% CI 1.4 – 38.3). In addition, 22.2% of the variation of p16 expression based on multivariate analysis demonstrating a significant correlation (p = 0.01). Conclusion: p16 expression with histopathology diagnostic characteristic in patient who underwent Loop electrosurgical excision procedure (LEEP) was found statistically significant. Moreover, clinical application of p16 in daily practice should be performed with consideration especially for pre-cancer lesion in LEEP biopsy specimen procedure and clinicopathological approach is essential.
