Abstract

Anesthetic management of obese pediatric patients is challenging. With increasing prevalence, more childwith severely obese and comorbidities present for surgery everyday. Definition of obesity is based on bodymass index percentiles spesific to age and sex. The precursors of adult obesity-related comorbidities are often seen in obese children. Respiratory and cardiovascular comorbidities increase perioperative risk in the severely obese child. Optimal and safe anesthetic management of an obese child requires thoughtful pre prosedure assessment and meticulous perioperative management tailored to associated comorbidities, with hieghtened wareness of potential perioperative complications. There are remains a need for improved guidelines for risk stratification, drug dosing and postoperative disposition in this patient population.

Abstract

Background: Brugada syndrome is a congenital disease characterized by Right Bundle Branch Block(RBBB) with an increase in the ST segment in the right precordial sadapans and is associated with suddendeath and lethal arrhythmias. Brugada syndrome is reported to be responsible for 4% of all sudden deaths and 20% of sudden deaths in those who do not suffer from structural heart disease and is the sadapaning cause of death in subjects under the age of 40 years. We reported a 40 years-old man with diagnosis of SO Filled Eye OD and was planned to underwent an OD Evacuation SO. There was Brugada ECG type 2 in this patient without any history and clinical symptoms of Brugada. Anesthesia was carried out with GA LMA Supreme No. 4 and induction of anesthesia with Sevoflurane inhalation agents. Monitoring during operations with 5 sadapan ECGs, NIBP, SpO2 and IOC. Patient’s hemodynamics during surgery was managed to be stable without any turmoil that can cause arrhythmias and VT VF. After the operation, monitoring in the recovery room was carried out for 4 hours and after being stabilized returned to the ward.

Abstract

Arnold–Chiari malformation (ACM) type I in syringomyelia patients can present with autonomic dysfunction.Anesthesia management requires careful preoperative assessment, precise identification of potential problems, and appropriate planning. We present a case report of a 29 years-old woman diagnosed with ACM type I with siringomielia associated with autonomic dysfunction. The patient was underwent suboccipital decompressive craniotomy. Patient exhibited symptoms of weakness in her upper limbs since 3 years ago, unable to hold items, paresthesia, and gradually progressed loss of temperature and pain sensation. Routine preoperative investigation were within normal limits. After intubation, the patient was placed in prone position. Thus, patient had hypotension with tachycardia, managed with intravenous (IV) fluid and vassopressor. During surgery the patient experiences instability of hemodynamics profile.

Abstract

Latar Belakang: Angka kejadian infeksi pada teknik epidural anestesia memang sedikit, namun beratnya komplikasi yang dapat ditimbulkan menjadi permasalahan yang harus dieliminasi faktor pencetusnya. Salah satu faktor yang berpengaruh adalah campuran obat yang aman secara mikrobiololgi. Pada sebagian praktek klinis di rumah sakit, pencampuran obat epidural dilakukan oleh petugas kesehatan melalui prinsip aseptik yang dilakukan pada ruangan tidak standar.Tujuan: Mengetahui keamanan mikrobiologi campuran obat epidural levobupivakain 0.125% dengan fentanil 1mcg/ml dalam NaCl 0.9%.Metode: Sebuah uji laboratoris deskriptif observasional pada 49 sampel berdasarkan convenient sampling. Sampel disimpan selama 3 hari pada suhu 40 celcius. Parameter mikrobiologi berupa pemeriksaan angka kuman dilakukan pada hari ke 0 dan ke 3.Hasil: Ditemukan kuman 2 CFU/mm pada 1 sampel di hari ke-3 penyimpanan. Pencampuran pada tempat yang tidak standar, dan efek antibakteri levobupivakain yang lebih rendah serta spektrum kuman yang lebih sedikit merupakan faktor risiko timbulnya kuman.Kesimpulan: Terjadi kontaminasi sebanyak 2,04% (1 sampel) pada pencampuran levobupivakain 0.125%, fentanil 1mcg/ml dan NaCl 0,9%, pada penyimpanan dalam lemari es sampai 3 hari.

Abstract

Intravenous fluid theraphy in acute ill patient has been the subject of health practices for more than 50 years. The development of knowledge in dealing with patients and illness has raised questions about the validity of Holliday Segar formula. Many researchers found hyponatremia in pediatric patient who were hospitalized and received intravenous fluid theraphy using the Holliday Segar formula. This thread tries to review the use of the Holliday Segar formula based on research studies that have been conducted.

Abstract

Background: Mortality predictor scores are important as prognosis tool for patients and families and to improve the quality of patient services in hospitals. RTS is easy to use and has been studied in several centers to estimate the prognosis of mortality in traumatic brain injury patients. Dr. Sardjito Hospital has never been investigated regarding the use of RTS mortality predictions in cases of traumatic brain injury. Objective: To validate the Revised Trauma Score (RTS) to predict mortality rates of traumatic brain injury patients in Dr. Sardjito General Hospital. Method: The study was conducted in the Medical Records Installation of Dr. Sardjito General Hospital Yogyakarta by searching the data of patients who were admitted to treatment at the Dr. Sardjito with a diagnosis of traumatic brain injury from 1 January 2018 to 31 December 2018 retrospectively. The RTS prediction number used to predict the number of deaths compared to the actual death rate of the sufferer group (observed death) with the Hosmer and Lemeshow (HL) test. Results: The number of inclusion samples in this study was 325 patients. Discrimination in this study obtained an AUC value of 0.030 (CI 95%: 0.012-0.048)]. The ability of RTS discrimination on the mortality of traumatic brain injury patients is 1-0.030 = 0.970 or 97%. With youden index, a cut of point for a significant RTS value with a mortality outcome for traumatic brain injury patients at Sardjito General Hospital 6.43 with a sensitivity value of 87% and a specificity of 94,6% and RR value of 32,73. HL test obtained p value = 0.976 (p> 0.05). Conclusion: The Revised Trauma Score (RTS) system has a very high discrimination ability and good calibration so that the Revised Trauma Score (RTS) has a high prognosis ability in predicting the mortality of patients with traumatic brain injury at RSUP Dr. Sardjito.

Abstract

Esophageal atresia with trachheoesophageal fistula occurs in 1 in 3,000 to 5,000 births. Esophageal atresia is often associated with other congenital anomalies like ventricular septal defects, patent ductus arteriosus or tetralogy of fallot. We reported a neonatal patient with esophageal atresia. Surgery was performed 2 days after being treated at the NICU. The operation lasted for 2 hours 25 minutes. The anesthetic technique is GA, semi open with Jackson Reese, ETT number 3, controlled breathing. The premedication was 0.1 mg atropine sulphate and 5 µg fentanyl, induction and maintenance were sevoflurane and O2, and with 1mg atracurium muscle relaxant. Postoperatively, patient was transported to the NICU in an intubated condition. The problems of anesthesia management on patients with esophageal atresia are evaluation of aspiration pneumonia, gastric overdistence, difficulty in ventilating the patient, the presence of other congenital abnormalities, and postoperative intensive care. Early recognition, prevention of aspiration, and immediate delivery to tertiary health care center will improve infant morbidity and mortality significantly.

Abstract

Opioids have become the flagship agents of modern perioperative care and pain management. The word opium comes from the Greek opion (“juice poppy”) or opium poppy (Papaver somniferum) is the source of 20 different alkaloids. Recent studies have shown that the role of opioid receptors on immune function is complicated, but they work through a variety of different mechanisms. Administration of various opioids shows different effects on the immune system: immunosuppressive, immunostimulatory, or both. Previously it was believed that most opioids suppress the immune system, but recent research suggests they may have a dual effect. However, the mechanism by which opioids and opioid receptors work in the immune system is still not clearly understood.

Abstract

Background: High dose opioid has been clinically proven to provide effective blockage of stimuli during head incision but it has several unwanted side effects. Scalp block is an alternative modality which has been used as an adjunct to general anesthesia aimed to blunt stress response throughout craniotomy while maintaining more stable condition in terms of less hemodynamic fuctuation. Objective: The study is aimed to compare fentanyl comsumption in general anesthesia combined with scalp block using saline and general anesthesia combined with scalp block using levobupivacaine 0.5% in patient which underwent craniotomy. Method: Twenty patients suffered from intracranial tumor with age ranging from 18 - 65 years who underwent craniotomy tumor removal at RSUP Dr. Sardjito were divided into 2 treatment groups in which the Levobupivacaine 0.5% block group received general anesthesia with a combination of scalp blocks using Levobupivacaine 0.5% while the control group received general anesthesia with a combination of scalp blocks using saline. Patients with local anesthetic allergy and uncontrolled bleeding were excluded from data collection. Result: The mean of fentanyl addition in patients with Levobupivacaine block was 513.0 mcg with a standard deviation of 164.2 mcg. Whereas in patients with Levobupivacaine block the average fentanyl addition was 1149.5 mcg with a standard deviation of 318.4 mcg. The mean difference in the addition of fentanyl between the two groups of 636.5 mcg showed a signifcant difference p = 0.001 (p <0.05). Conclusion: Combination of scalp blocks using Levobupivacaine 0,5% with general anesthesia showed reduction of fentanyl consumption alongside with statistically insignifcant hemodynamic fuctuation compared to general anesthesia and saline block in supratentorial brain tumor patient undergoing craniotomy. Hemodynamic profle in Levobupivacaine 0,5% group was maintained better than control group.

Abstract

Background: Geriatrics is defined as someone who reaches the age of 60 years and over. With the increase in the level of the economy and health, the geriatric population will increase. So that surgery and anesthesia in geriatric patients also increased. Geriatric patients tend to have more comorbidities accompanied by a decrease in physiological function and cognitive function so that it has an impact on mortality and morbidity. Objective: To identify the factors related to mortality and morbidity in geriatric patients undergoing anesthesia procedures at Dr. Sardjito General Hospital. Method: This study was conducted in a prospective analytical observational to identify risk factors in geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital. The affordable population was all geriatric patients aged 60 years and over, who underwent anesthesia in the operating room and the time of the study was carried out for 3 months. In this study, variables that are thought to have a relationship with mortality and morbidity of geriatric patients will be tested using a univariate test using the Student's t-test if the data is numerical, while if the data is categorical using the Chi Square test. If the results of p<0.25 will be continued with the logistic regression test of each variable. And if p<0.05 then the variable is included in the multivariate analysis and if p<0.05 is considered significant. Results: The factors identified as factors affecting mortality of geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital are age ≥75 years, dementia, and digestive surgery with p <0.05 in both univariate and multivariate tests. In the multivariate logistic regression test, factors affecting morbidity of geriatric is digestive surgery with p < 0,05. Conclusion: Patients’ age≥75 years, dementia and digestive surgery are the identified factors affecting mortality and morbidity of geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital.
