Abstract

Stroke non-hemoragik (SNH) terjadi akibat adanya hambatan aliran darah ke otak yang disebabkan oleh sumbatan pada pembuluh darah, sehingga mengganggu fungsi neurologis secara parsial maupun menyeluruh. Penulisan karya ilmiah ini bertujuan untuk mengkaji pemberian asuhan keperawatan pada pasien dengan masalah psikologis berupa ansietas. Metode  yang digunakan adalah studi kasus, dengan subjek satu pasien stroke non-hemoragik yang sedang menjalani perawatan di ruang stroke unit dan menunjukkan gejala ansietas. Pengumpulan data dilakukan melalui wawancara, observasi, serta peninjauan dokumentasi medis. Dalam proses keperawatan yang berlangsung selama tiga hari, pasien menunjukkan tanda-tanda kecemasan yang berkaitan dengan gangguan mobilitas pada sisi kanan tubuh. Intervensi yang diberikan untuk menangani ansietas tersebut berupa terapi imajinasi terbimbing, sebagai bentuk pendekatan keperawatan berbasis bukti (Evidence-Based Nursing). Hasil evaluasi menunjukkan bahwa setelah dilakukan terapi selama tiga hari, tingkat kecemasan pasien mengalami penurunan yang signifikan dan masalah keperwatan apat teratasi. Penerapan (EBN) yang diterapkan menunjukan hasil yang cukup baik, adanya penurunan terhadap tingkat ansietas, pasien sudah tidak cemas lagi dan pasien sudah tampak tenang.  Kesimpulannya, terapi imajinasi terbimbing efektif digunakan untuk mengurangi ansietas pada pasien stroke non hemoragik.

Abstract

Stroke non-hemoragik terjadi akibat penyumbatan pembuluh darah yang menyebabkan gangguan aliran darah ke otak, sering menimbulkan masalah mobilitas fisik pada pasien. Penelitian ini bertujuan menggambarkan pengaruh efektivitas terapi Range of Motion (ROM) menggunakan bola karet dalam meningkatkan kekuatan otot pasien Stroke Non Hemoragik (SNH) dengan gangguan mobilitas fisik. Metode yang digunakan adalah studi kasus dengan satu responden yang mengalami penurunan kekuatan otot, dimana intervensi berupa terapi ROM aktif dan didapatkan hasil pengakajian yaitu sulit menggerakkan ekstremitas kiri. Pengukuran kekuatan otot dilakukan menggunakan Manual Muscle Testing (MMT) sebelum dan sesudah terapi. Hasil evaluasi pada hari ketiga setelah 3 hari latihan dengan frekuensi latihan menggenggam bola karet 2 kali sehari dengan durasi waktu 10 – 15 menit atau setara dengan 6 kali pertemuan menunjukkan keluhan susah menggerakkan ekstremitas kiri menurun dan adanya peningkatan kekuatan otot dari sebelum dilakukan terapi menggenggam bola karet yang dirasakan pasien penurunan kekuatan otot skor 3 dan setelah diberikan terapi menggenggam bola karet meningkat menjadi kekuatan otot skor 4. Simpulan penelitian ini adalah terapi ROM dengan bola karet efektif meningkatkan kekuatan otot pada pasien Stroke Non Hemoragik (SNH) sehingga dapat menjadi bagian dari asuhan keperawatan untuk mengatasi gangguan mobilitas fisik.

Abstract

Patient  with congestive heart failure(CHF) often experience shortness of breath due to fluid buildup in the lungsIn the Emergency Department, shortness of breath is a primary complaint requiring prompt and appropriate treatment. Incorrect body positioning can exacerbate symptoms. The semi-Fowler's position is a simple intervention that can increase lung expansion and decrease heart workload. The purpose of this studyTo determine the effectiveness of the semi-Fowler position in reducing shortness of breath in congestive heart failure (CHF) patients in the Emergency Department. The focus of the study is highly relevant to emergency clinical conditions and contributes to nursing practice in the ER, particularly in the initial management of shortness of breath symptoms due to CHF. The study used a case study design, which is appropriate to describe the direct effects of nursing interventions on individual patients in acute conditions. The duration of the intervention (1x7 hours) and respiratory rate measurements are appropriate steps to evaluate clinical changes. The results showed a decrease in respiratory rate from 30x/minute to 24x/minute after the semi-Fowler position intervention, which indicates an improvement in respiratory conditions, indicating that the breathing pattern was partially resolved, meaning the intervention provided benefits but had not completely resolved the problem of shortness of breath. These results are in line with the physiological theory that the semi-Fowler position can reduce diaphragmatic pressure and increase lung expansion.

Abstract

Ischemic stroke or non-hemorrhagic stroke (SNH) is a condition where there is a blockage of blood vessels that causes blood flow to the brain to partially or completely stop. This final scientific work aims to assess the effectiveness of music therapy in non-hemorrhagic stroke (SNH) patients with anxiety nursing problems. The method applied in the preparation of this final scientific work is a single case study method. The subject in this case study involves one client with a case of non-hemorrhagic stroke (SNH) and has an anxiety problem who is undergoing treatment. Data collection using interview techniques, observation and documentation. The nursing care process for clients is carried out for 3 days, and the results of the assessment are obtained, namely the stiff body on the left so that the patient experiences anxiety (anxiety) due to the conditions experienced. The intervention provided is the reduction of anxiety combined with the application of EBN, namely music therapy which is implemented for 15 minutes for 1 time in 3 days. Based on the evaluation results, it was found that on the third day the nursing problem of anxiety had been resolved with objective data the patient looked calm. Furthermore, it can be concluded that music therapy can reduce the level of anxiety in non-hemorrhagic stroke (SNH) patients. Music therapy interventions can be included as part of the nursing SOP or supporting therapy.

Abstract

Chronic Kidney Disease (CKD) is a decline in kidney function that occurs gradually over a long period of time and will get worse over time. One of the clinical symptoms of CKD is shortness of breath which is a lack of oxygen entering the lungs. Sem-fowler position as one of the nursing actions that can reduce shortness of breath so that oxygen intake increases and shortness of breath decreases. This study aims to analyze nursing care by giving sem-fowler position to CKD patients with nursing problems of gas exchange disorders. This research method uses a case study method conducted from January 30 - February 01, 2025. The subject in this scientific work involves 1 patient with CKD cases.  Data collection using interview techniques, observation, and physical examination. Implementation carried out for 3 consecutive days, accompanied by measurement of peripheral capillary oxygen saturation (SpO2) using a Mindray PM 60 Oxymeter. The characteristics of this case study patient are a 61-year-old female patient and experiencing end-stage CKD (Stage V). Based on the results of oxygen saturation values before application with a value of 94% and after application including the normal category with a value of 98%. The conclusion of the application of the semi-fowler position is able to increase oxygen saturation in CKD patients.
