Abstract

Nyeri neuropati merupakan kondisi kronis akibat disfungsi sistem saraf somatosensori yang memerlukan pendekatan terapi multimodal untuk mengoptimalkan manajemen nyeri. Pregabalin, sebagai agen farmakoterapi utama, bekerja dengan menurunkan eksitabilitas neuron melalui modulasi kanal kalsium tipe alfa-2-delta, menunjukkan efektivitas dalam mengurangi gejala nyeri neuropati dengan profil farmakokinetik yang lebih unggul dibandingkan gabapentin. Pendekatan nonfarmakologis, seperti cognitive behavioral therapy (CBT) dan stimulasi listrik noninvasif, juga berperan dalam meningkatkan kualitas hidup pasien. Dengan berkembangnya penelitian terhadap agen terapi baru seperti cannabinoid dan mirogabalin, serta pendekatan berbasis personalisasi, strategi manajemen nyeri neuropati terus mengalami inovasi untuk meningkatkan efektivitas dan tolerabilitas terapi.

Abstract

Chronic pain is a common and distressing problem for the population. The cyclooxygenase (COX)-2 selective and nonselective nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used in managing acute and chronic pain. Research has provided a greater understanding of the risks of gastrointestinal, cardiovascular, and renal events associated with NSAID use. Proton pump inhibitors (PPIs) are widely prescribed due to their gastroprotective effects in NSAID users. However, PPIs also have several acid-inhibition-related and unrelated adverse effects that limit their use in high-risk patients. We report five patients suffering from painful inflammatory pain (flare rheumatoid arthritis, painful knee bursitis, and osteoarthritis). In this case series we evaluate the use of DLBS2411 as an alternative to PPIs. Our study suggests that DLBS2411 can be a safe alternative to PPIs for gastrointestinal high-risk patients who require anti-inflammatory agents.

Abstract

Introduction: Herpes zoster develops in individuals who have previously had chickenpox because both conditions arecaused by the same virus, the varicella zoster virus (VZV). After recovering from chickenpox, the virus remains dormant in the sensory ganglia and can reactivate when the immune system becomes weakened. Case: A 13-year-old girl presented with complaints of reddish bumps, which are reported to be itchy and painful, lasting for approximately 4 days. This patient has no history of varicella disease but exhibits symptoms of herpes zoster. Treatment is provided to suppress the progression of the disease, as well as to manage and prevent further complications. Conclusion: Herpes zoster ophthalmicus (HZO) results from the reactivation of the VZV in the ophthalmic branch of the trigeminal nerve. Individuals with weakened immunity, particularly older adults, are at higher risk. HZO can lead to various ocular complications such as conjunctivitis, blepharoconjunctivitis, keratitis, and uveitis. Early antiviral treatment can reduce the severity and duration of symptoms, prevent complications, and minimize the risk of postherpetic neuralgia. Proper management, including antiviral therapy, pain control, and eye care, is crucial to improve clinical outcomes.

Abstract

Background: Highly active multiple sclerosis (HAMS) is a severe phenotype of multiple sclerosis (MS), accounting for 10% of all relapsing-remitting MS (RRMS) cases, characterized by rapid clinical decline and disease progression despite treatment with disease-modifying therapy (DMT). It is associated with high morbidity and mortality rates, thus requiring different therapeutic approaches. We report a case of DMT switch in a 30-year-old female with HAMS. Case: A 30-year-old female presented with a 3-month history of numbness and tingling on her fingers, back, and lower extremities. Examination revealed hyperreflexia and positive Hoffman-Tromner sign. Brain magnetic resonance imaging (MRI) revealed multiple demyelinating lesions suggestive of MS, including Dawson’s fingers. She was diagnosed with MS and initially treated with subcutaneous (SC) interferon beta-1a (IFNβ-1a) 22 mcg, three times weekly. As the patient continued to experience relapses and develop new lesions on biannual repetition of scans, she was diagnosed with RRMS. Four years after the initial diagnosis, her relapses became more frequent with incomplete recovery in between, suggestive of HAMS. Therapy was subsequently switched from IFNβ-1a to cladribine. Conclusion: Two approaches in the treatment of MS are the escalation and induction therapy. To date, no standardized treatment protocols for HAMS have been established. Patients with HAMS are candidates for the induction approach due to the narrow window of opportunity for treatment and the risk of early disease progression. Despite the risk of developing adverse reactions from being on potent immunosuppressive drugs, the imminent neurological disability that comes with HAMS outweighs this risk. The use of immune-reconstitution DMTs helps to achieve no evidence of disease activity (NEDA). Cladribine has shown promising outcomes in the treatment of HAMS and is frequently used for induction therapy.

Abstract

Biologi spasial telah muncul sebagai bidang transformatif, memberikan wawasan yang belum pernah terjadi sebelumnya tentang organisasi spasial biomolekul, sel, dan jaringan. Ulasan ini mengkaji kemajuan dalam multiomik spasial antara tahun 2020 hingga 2025, dengan fokus pada teknologi utama, termasuk imunofluoresensi multipleks (multiplex immunofluorescence/mIF), mikroskopi lembaran cahaya, dan transkriptomik spasial. Inovasi-inovasi ini memungkinkan analisis multidimensi resolusi tinggi terhadap sistem biologis yang kompleks, merevolusi penelitian kanker, studi mikrobioma, dan ilmu saraf. Integrasi antara omik sel tunggal dan spasial telah memberikan pemahaman yang lebih dalam mengenai heterogenitas tumor, distribusi sel imun, dan interaksi seluler dalam lingkungan mikro tumor. Selain itu, multiomik spasial telah memperluas aplikasi di luar onkologi, menawarkan wawasan baru dalam biologi liver, sirkuit saraf, dan mikrobioma usus. Namun demikian, dengan adanya kemajuan ini, tetap terdapat tantangan dalam analisis data, integrasi, dan aksesibilitas. Evolusi berkelanjutan alat komputasi, analisis berbasis kecerdasan buatan, dan integrasi multiomik akan semakin meningkatkan dampak bidang ini, serta membuka jalan bagi pengobatan yang lebih personal dan terapi presisi.

Abstract

Mitokondria, pabrik energi sel, memainkan peran penting dalam menjaga fungsi neuron dan sirkuit melalui produksi adenosine triphosphate (ATP) melalui fosforilasi oksidatif (oxidative phosphorylation/OXPHOS). Dalam otak mamalia yang menua, efisiensi OXPHOS menurun, menyebabkan disfungsi mitokondria, stres oksidatif, dan penurunan kognitif. Penelitian terbaru menyoroti hubungan antara eksitasi neuron dan transkripsi gen mitokondria, menunjukkan bahwa keterkaitan ini berkurang seiring bertambahnya usia. Studi lain memperkuat temuan ini dan menawarkan perspektif yang lebih luas tentang mekanisme yang mendasari disfungsi mitokondria pada penuaan serta strategi terapeutik potensial. Ulasan ini mengkaji penelitian-penelitian mutakhir untuk memberikan pemahaman komprehensif tentang penurunan mitokondria terkait usia dan dampaknya terhadap fungsi kognitif.
