Abstract

Post-operative pain after upper extremity ORIF is complex, involving tissue damage, inflammation and nociceptive mechanisms that require a comprehensive pharmacological approach. The intensity of post-ORIF pain typically ranges from moderate to severe in the first 48-72 hours and can affect early mobilisation, respiratory function and the patient's physiological stress response. Research objective: To determine the pattern of analgesic use for pain management after upper extremity ORIF surgery under general anaesthesia at Pandan Arang Boyolali Hospital. Research Methods: This is a qualitative research with case study approach. Data was collected using quota sampling technique. The sample in this study amounted to 5 patients with inclusion criteria: upper extremity orthopaedic surgery patients who will undergo ORIF in the Central Surgical Installation of Pandan Arang Boyolali Hospital in April 2025, patients with general anaesthesia Laryngeal Mask Airway (LMA). Results: Based on the results of observations, it was found that the pattern of use of pharmacological pain management in patients after upper extremity ORIF surgery with general anaesthesia at Pandan Arang Boyolali Hospital was the administration of 100 mcg fentanyl and 4 mg ondansentrone dripped in 500 ml Tutofusin infusion for surgery with a duration of > 1 hour and a large incision area.

Abstract

Paracetamol is a widely used analgesic and antipyretic drug in tablet dosage form. Ensuring the stability of paracetamol tablets is essential to maintain drug efficacy, safety, and quality during their shelf life. This study aims to evaluate the physical and chemical stability of paracetamol tablets stored under different temperature and humidity conditions. The review method used in this article involves analyzing published data from several sources, focusing on changes in physical appearance (color, shape, and odor) and chemical parameters such as active pharmaceutical ingredient content over time. The findings indicate that higher temperatures and humidity accelerate degradation and affect the drug's quality, particularly through discoloration and reduction in active content. Packaging also plays a crucial role in maintaining stability, especially in tropical climates. The results highlight the importance of stability testing in pharmaceutical quality assurance and support the application of Good Manufacturing Practices (GMP) and appropriate storage guidelines. This paper contributes to the understanding of stability profiles of commonly used drugs and provides a reference for the formulation and packaging development of heat-sensitive medications.

Abstract

Coping ability is the ability to cope with pressure and problems. there are several types of coping strategies, including passive and active coping. The purpose of this study is to see how social cognitive theory can be applied in guidance and counseling practice to improve adolescents' ability to handle stress. To find the application of the theory in guidance and counseling, relevant literature has been collected and analyzed using the literature review method. Results show that the principles of Cognitive Social Theory, which include strong social support, the use of active coping strategies, and parental involvement, contribute positively to the development of adolescents' coping skills. The results showed that adolescents who participated in a TSC-based guidance and counseling program showed better emotional resilience and self-efficacy. It was found that strengthening coping skills requires support from peers and family. To ensure that the intervention meets the needs of adolescents, ongoing evaluation is also needed. This study points out some issues when implementing a TSC-based guidance program, such as obtaining adequate training for counselors and ensuring that resources are available. It is expected that adolescents will be able to gain better skills to handle diverse situations and improve their emotional resilience by implementing an effective theory-based approach. Further research is needed, especially to improve adolescents' adaptability in today's increasingly complex environment.

Abstract

Adolescent Idiopathic Scoliosis (AIS) is the most common progressive musculoskeletal disorder among adolescents, characterized by a lateral curvature of the spine with an unknown cause. This condition often results in postural asymmetry, chronic low back pain, and reduced mobility. This case report involves a 17-year-old female high school student diagnosed with AIS, presenting with lower back pain and functional limitations. The physiotherapy intervention included three sessions over two weeks, utilizing Microwave Diathermy (MWD) as a physical modality, along with Schroth method and strengthening exercises. The outcomes demonstrated a reduction in pain intensity, improved muscle strength, increased spinal mobility, and enhanced thoracic expansion. Additionally, the patient showed an improvement in quality of life as measured by the Scoliosis Research Society-22 (SRS-22) questionnaire. These findings suggest that structured conservative physiotherapy, combining physical modalities and specific exercises, is effective in alleviating clinical symptoms, improving postural awareness, and supporting the functional well-being of AIS patients. This case highlights the importance of early and consistent physiotherapeutic intervention to manage symptoms and potentially slow curve progression.

Abstract

Optimization of primary and secondary packaging is crucial for the quality and efficiency of drug production. The drug packaging process consists of primary packaging that is in direct contact with the product and secondary packaging that provides additional protection and important information. This study aims to analyze various methods applied in pharmaceutical packaging to assess the effectiveness of Lean Manufacturing (VSM), Single Minute Exchange of Dies (SMED), and Overall Equipment Effectiveness (OEE). This study shows that VSM significantly reduces the time of secondary packaging activities. SMED reduces the lead time of primary packaging analysis by 58.06%. Meanwhile, OEE analysis of primary packaging identified high set-up/adjustment as the main cause of losses. The importance of implementing structured methods to increase productivity, reduce waste, and ensure a more efficient and effective packaging process in the pharmaceutical industry, contributing to improving product quality and safety.

Abstract

Carpal Tunnel Syndrome (CTS) is a peripheral neuropathy disease that occurs due to compression or pressure on the median nerve which causes narrowing of the carpal tunnel. Common symptoms of CTS are pain, numbness, and paresthesia which usually spread to the fingers, especially the thumb, index finger, and middle finger. To determine the effectiveness of non-operative therapy in cases of Carpal Tunnel Syndrome using modalities of tools and exercise therapy in the form of nerve gliding exercise, free exercise with resistance, hand grip, and manual therapy techniques. Case report study is used as a research method by taking samples from 1 patient with a case of Carpal Tunnel Syndrome (CTS). The measuring instruments used are Visual Analog Scale (VAS) for the pain scale, Manual Muscle Testing (MMT) to measure muscle strength, Goniometer to measure Range Of Motion (ROM), and Wrist Hand Disability Index (WHDI) to measure functional activity ability. After the implementation of the 3rd therapy there were changes, namely decreased pain, increased muscle strength, increased ROM and increased functional activity.  This case study shows that there is effectiveness in providing exercise therapy, manual therapy and electrotherapy in cases of Carpal Tunnel Syndrome (CTS).

Abstract

The management of pharmaceutical and medical waste, particularly expired or damaged drugs, remains a challenge across various healthcare facilities. A study at Grade II R. W. Mongisidi Hospital in Manado showed a 76% compliance rate with the Ministry of Health’s 2021 guidelines, while Mamuju Regional Hospital failed to meet the standards outlined in Regulation No. 1204/2004. In community pharmacies in East Bandung, most facilities disposed of drugs independently, with only 59.6% following proper waste management procedures. These findings highlight the need for improved supervision, education, and adherence to standardized guidelines to minimize the negative impacts on health and the environment.
