Abstract

Effect in Pain Scale of Trocar Insertion, Onset, and Duration of Anesthesia of Subtenon Anesthesia without Premedication Compared with Premedication in Pars Plana Vitrectomy: A Randomized Controlled Trial. Background: Pars plana vitrectomy (PPV) has traditionally been performed under general anesthesia. However, in recent years, regional anaesthetic techniques such as subtenon block have gained popularity due to their safety and effectiveness, reducing the risk of severe complications associated with needle-based blocks. Patients and Methods: This randomized controlled trial (RCT) was conducted between November 2023 to March 2024 at a tertiary teaching hospital in Indonesia. A total of 30 patients undergoing vitrectomy were randomized assigned to one of two groups: with premedication or without premedication. The primary outcomes evaluated were the pain scale during trocar insertion, the onset of anesthesia, and the duration of anesthesia. Statistical analyses were performed using the Mann-Whitney U test and ANCOVA. Results: No significant differences were observed between the two groups regarding the pain scale during trocar insertion, the onset of anesthesia, or the duration of anesthesia (p<0.001). However, after adjusting for diagnosis and type of the therapy, the group without premedication demonstrated a significantly longer duration of anesthesia. Conclusion: Subtenon anesthesia without premedication represents a viable alternative for pars plana vitrectomy. It offers practical benefits, such as eliminating the need for fasting and intravenous line placement, while maintaining patient and operator comfort.

Abstract

Diabetes mellitus is a major cause of multisystemic complications, particularly in patients with poor glycemic control since a young age. Chronic hyperglycemia induces vascular, neurological, and renal damage, predisposing patients to severe functional impairments and infections. Case Report: We present a 21-year-old male with long-standing uncontrolled diabetes mellitus who developed compressive myelopathy, diabetic kidney disease (DKD), diabetic neuropathy, and neurogenic bladder. The patient exhibited progressive lower limb weakness, urinary and fecal incontinence, chronic back pain, severe hyperglycemia, electrolyte imbalances, and a significant decline in renal function. Discussion: This case illustrates the complex interplay between diabetic microvascular and neurological complications. Chronic hyperglycemia led to glomerular hyperfiltration and progressive renal injury, while peripheral and autonomic neuropathy resulted in debilitating sensory-motor symptoms and recurrent urinary tract infections. The overlapping pathologies highlight the necessity for early screening, aggressive metabolic control, and a multidisciplinary approach to management. Conclusion: Early diagnosis and prompt multidisciplinary intervention are critical to prevent irreversible multisystem damage in patients with longstanding diabetes. Comprehensive management strategies focusing on infection control, renal protection, neuropathic pain relief, and glycemic stabilization are essential to optimize patient outcomes.

Abstract

Generalized Anxiety Disorder is a common and disabling illness that is often underdiagnosed and undertreated, characterized by chronic, pervasive anxiety and worry accompanied by nonspecific physical and psychological symptoms including restlessness, fatigue, difficulty concentrating, irritability, muscle tension, and sleep disturbances. While anxiety can be an appropriate response to stressful situations, it is considered pathological when disabling and difficult to control. This case study, which aimed to explore generalized anxiety disorder and its management in patients, utilized diagnostic criteria from Pedoman Penggolongan dan Diagnosis Gangguan Jiwa-III and the Diagnostic and Statistical Manual of Mental Disorders-5, finding that the patient's manifestations aligned with established diagnostic criteria. Generalized anxiety disorder often goes undiagnosed and untreated due to accompanying physical complaints, requiring not only pharmacological interventions but also non-pharmacological approaches such as cognitive behavioral therapy to effectively control the patient's thought patterns.

Abstract

The digitization of registration services is part of the modernization of healthcare services to speed up administrative processes and improve patient comfort. Hospital X developed the PANGESTU application as an online reservation system for outpatients. Assessing the effectiveness of the PANGESTU application strategy in optimizing the online reservation process and identifying supporting and inhibiting factors in its implementation. The research used a qualitative descriptive approach through observation, interviews with registration officers, security guards, and patients, as well as documentation over a period of six months. The application speeds up the registration process and makes it easier for patients who are familiar with technology. However, obstacles were encountered, such as digital literacy gaps, the illegibility of reservation data, and the lack of formal training for officers. The success rate of an application is highly dependent on the readiness of human resources and the adaptability of the system to user variations. Data integration and digital education are key factors for optimization. The PANGESTU application has successfully created service efficiency, but it is not yet fully inclusive. Technical optimization, improved digital literacy, and active involvement of human resources are necessary to ensure the sustainability of the system.

Abstract

This study aims to analyze the impact of the implementation of Electronic Medical Records (EMR) on the effectiveness of health services at the West Java Provincial Occupational Health Hospital. The issues raised relate to the effectiveness of the service system, information integration, and the readiness of human resources in facing digital transformation. The method used was a qualitative approach, with data collection techniques through in-depth interviews with health workers, administrative staff, and heads of medical record installations. The results of the study indicate that the implementation of EMR has had a significant impact on improving data access speed, integration between service units, and the accuracy of medical documentation. This system also accelerates administrative processes and clinical actions, reduces the risk of data loss, and strengthens coordination between professions within the hospital environment. However, the implementation of EMR still faces technical challenges such as network disruptions and limitations in supporting infrastructure. From a human resources perspective, adaptation to the new system has been relatively smooth, supported by ongoing training and guidance provided by the hospital. Overall, EMR has proven to be a strategic step toward achieving more efficient, integrated, and data-driven healthcare services in the digital age.

Abstract

Patient adherence to treatment is an essential component in achieving successful therapy, especially in the management of chronic diseases that require long-term treatment and active patient involvement. Non-adherence can lead to therapy failure, increased complications, higher health care costs, and decreased quality of life. This study aims to analyze factors that influence patient adherence to treatment at Hospital X using a descriptive quantitative approach. The study sample consisted of 100 outpatients selected purposively and analyzed using cross-tabulation and chi-square statistical tests. The results of this study indicate that there are still respondents who are not compliant with treatment. Variables that show a significant relationship to compliance are education level (p = 0.034) and family support (p = 0.002). In contrast, gender (p = 0.859) and age (p = 0.640) do not have a significant relationship. These findings underline the importance of education-based interventions that are tailored to the patient's educational background, as well as active family involvement as an integral part of the care process. This study recommends a more holistic service approach, which not only emphasizes clinical aspects, but also considers social, psychological, and cultural factors of patients as important determinants of compliance behavior.

Abstract

Hypertension is a major risk factor for cardiovascular disease and remains a persistent challenge in health systems, including in Indonesia. The Chronic Disease Management Program (Prolanis) under the National Health Insurance scheme (Jaminan Kesehatan Nasional, JKN) aims to improve the control of chronic diseases. However, the achievement of blood pressure control indicators at a clinic affiliated with JKN remains suboptimal. This study aimed to identify factors associated with blood pressure control among hypertensive patients enrolled in the JKN program. A cross-sectional quantitative design was applied to 120 hypertensive patients selected through quota sampling. Data were collected using standardized questionnaires (IPAQ, PSS-10, MMAS-8, and MSPSS) and analyzed using Chi-Square tests and multiple logistic regression. Results showed that 50.8% of respondents had controlled blood pressure. Among all variables examined, only physical activity was statistically associated with blood pressure control (p = 0.020; OR = 9.5; 95% CI: 1.74–51.53; p = 0.009). These findings highlight the importance of strengthening promotive and preventive approaches—particularly through structured physical activity and health education—as part of the Prolanis service to support sustainable blood pressure control.
