Abstract

One of the most common infections of the eyelid and periorbital soft tissues is preseptal cellulitis that is characterized by acute eyelid erythema and edema. Preseptal cellulitis are much more common in young children than adults. In addition, preseptal cellulitis is more frequent than orbital cellulitis. Preseptal cellulitis occurs anterior to the orbital septum. Preseptal cellulitis is a common infection of the eyelid and periorbital soft tissue that is characterized by acute erythema and edema of the eyelid.Most preseptal cellulitis is caused by local spread of sinusitis or adjacent dacryocystitis, external eye infection, or after trauma to the eyelidPreseptal cellulitis generally occurs in children rather than adults.Radiological examination such as imaging with MRI or CT is not indicated unless suspected orbital cellulitis or eyelid abscess is present, or there is a failure to respond to therapy.Administration of antibiotics in patients with preseptal cellulitis caused by bacterial infection must be started as soon as possible.This study discusses the case of girl with preseptal cellulitis who were treated with appropriate antibiotics.

Abstract

Diabetes mellitus (DM) is a group of metabolic disorders characterized by hyperglycemia. Currently DM has become a global health threat. 415 million people worldwide suffer from DM in 2015 and are expected to increase to 642 million by 2040. Application of family doctor based services based on evidence based medicine in patients by identifying risk factors, clinical problems, and patient management based on the framework of solving patient problems with approached and family approached patient. Analysis of this study is a case report. Primary data is obtained through history taking, physical examination and home visits to supplement family data, psychosocial and environmental data. Assessment is based on a holistic diagnosis from the beginning, the process, and the end of the study quantitatively and qualitatively. Patient ny. S 56 years old was diagnosed with Type 2 Diabetes Mellitus. Patients had internal risk aspects such as obesity, non-physical activity, offspring of diabetes mellitus, history of DM drug breakdown, curative treatment patterns. The external risk aspect of the patient is that the family does not understand the patient's disease and its complications, the family economy is low. Furthermore, the management consists of education about DM and its care for patients and families as well as initial GDS measurements. After evaluating, a decrease in GDS is obtained in patients.

Abstract

Rheumatoid Arthritis (RA) is an autoimmune disease characterized by erosive, symmetrical synovitis and requires long-term treatment so family support is needed in its management. This study is a case report. Primary data obtained through history taking, physical examination, and home visits. Secondary data were obtained from the patient's medical record. At the first visit, the patient's complaint was pain in the proximal interphalangeal joint (PIP) digital I-V dextra et sinistiti. Internal risk factors namely lack of knowledge about the causes of RA, precipitating factors, how to deal with acute symptoms, diet and exercise are recommended. External risk factors in patients are families who pay less attention to diet and exercise recommended to patients and do not use shoes and waterproof gloves and do repetitive movements without resting when farming. The second visit was to be intervened with the family conference method using poster media. The third visit found complaints of reduced joint pain because the patient took piroxicam 10 mg and applied liniment to the painful location. Patients have been consuming fruit and milk regularly and exercising muscle stretches three times a week for 30 minutes. Families remind diet and exercise accordingly. When farming use shoes and waterproof gloves and rest between repetitive movements. There is an increase in patient knowledge about the causes, precipitating factors, how to deal with acute symptoms, diet, and exercise that is appropriate for RA, characterized by an increase in post-test scores of 40.

Abstract

MRSA infection is an urgent global health problem. Synthetic or natural antibacterials should be developed as an effort to overcome the problem. The mauli banana peel contains potential antibacterial compounds such as flavonoids, tannins, saponins, alkaloids, and terpenoids. The purpose of this study is to determine the antibacterial activity of mauli banana peel extract (Musa acuminata) against methicillin resistant Staphylococcus aureus (MRSA). The design of this study was true experimental descriptive, with a post-test control group design. This study examined the antibacterial activity of mauli banana peel extract at concentrations of 100%, 50%, 25%, 12.5%, and 6.25% against MRSA bacteria by measuring inhibitory zone, minimum inhibitory concentration (MIC), and minimum bactericidal concentration (MBC). The results of this study showed that mauli banana extract has antibacterial activity against MRSA with the average diameter of inhibition zone ranging from 23,963 mm to 11,075 mm. The MIC obtained at concentrations of 6.25% and MBC at extract with concentration 50%. One-Way Anova test obtained p value = 0,000 (p <0.05) which means there is a significant difference. Based on the experiment, we can conclude that there is antibacterial activity of mauli banana peel extract (Musa acuminata) against methicillin resistant Staphylococcus aureus (MRSA).

Abstract

Malaria is a life-threatening infectious disease caused by parasites that are transmitted to people through the bites of infected female Anopheles mosquitoes or through blood transfusion. Malaria is an endemic disease that still remains a major health problem in the world. The authors report a case of 21-years old man who presented with intermittent fever and pancytopenia without any signs of spontaneous bleeding that had a history of travel to a malaria endemic area. The diagnosis of malaria is made through examination of peripheral blood morphology. These findings highlights the importance for clinicians to consider tropical malaria diagnosis in patients with acute infection and presented with pancytopenia. Management of this patient included administration of anti-malaria drugs, transfusion of PRC, enteral nutrition, adequate fluids administration, antibiotics in the condition of leukopenia, and administration of other symptomatic therapies. The clinical condition and laboratory findings gradually improved after six days of hospitality.

Abstract

Labor wiith history rupture of the membrane is at high risk for maternal and infant death, which is one of the causes of parity. Parity is one of the factors occurs premature rupture of membranes where maternal women with multiparity tend to be more susceptible to experience premature rupture of membranes. The purpose of this studyto determine the relationship between parity and PROM during inpartu.This research used observational analytic method with cross sectional design. A total sample of 249 patients were selected with consecutive sampling technique. Data collected from patient's medical record in obstetric room of RSUD Dr. H. Abdul Moeloek Lampung Province. The inclusion criteria of this research is mothers with premature rupture of membranes both vaginal delivery and cesarean section. Patients were accompanied by parity data inaccuracies, and damaged medical records were excluded from the study sample.The results showed that as many as 59 patients (23.69%) experienced with rupture of the membrane and as many as 31 (12.44%) patients who experienced rupture of the membrane with multiparity. Based on Chi-square test, p = 0.031 (p <0.05) was obtained. The conclusion of this study that there is relationship between parity with premature rupture membrane in RSUD Dr. H. Abdul Moeloek.

Abstract

Herpes shingles (HZ) is an infectious disease caused by the reactivation of the varicella shingles virus (VVZ). Are localized, mainly attacking adults with a characteristic of radicular pain, unilateral, and a horde of vesicles that are dispersed according to dermatoms that are ineralized by a single sensory nerve ganglion. Factors that potentially lead to reactivation are: previous exposure to VVZ (chickenpox, vaccination), age more than 50 years, Immunocompromais state, immunosuppressive drugs, HIV/AIDS, bone marrow transplantation or organ, violence, therapy Long-term steroids, psychological stress, trauma and the act of Distinction. Incidence of HZ in children 0.74 from 1000 person per year. This incidence increased to 2.5 from 1000 people at age 20-50 (adult age), 7, from 1000 people aged more than 60 years (older adult age) and reached 10 from 1000 person per year at the age of 80. Case: reported case of Mrs. P, 55 yo with a diagnosis of herpes shingles with TB MDR with based on anamnesis and physical examination administered therapy acyclovir tab 5x800 mg PO/day for 7 days, Paracetamol tab 3x500 mg PO/day if necessary, powder salicyl 2% applied on the lesions 3x/day and given the drug TB MDR namely Cycloserin, etionamides and moxifloxacin. Conculsion: The patient showed good therapeutic results.

Abstract

Stroke is the second leading cause of death and the third cause of disability in the world. Globally, 87% of strokes related to death and disability occur in low-medium income countries. Recovery from a stroke depends on many factors including the risk, the accuracy and speed of management, the disease that aggravates stroke and treatment and the implementation of early mobilization to prevent one complication of prolonged bed rest. Things to consider in handling post-stroke patients is family support. Family support can support patient recovery and recovery. The study conducted was a case report. Primary data was obtained through history taking and physical examination by making home visits. The assessment was carried out based on a qualitative initial holistic diagnosis, process, and end of visit. Based on the examination, the patient was a 62-year-old woman diagnosed with stroke, pressure sores, hypercholesterolemia, history of hypertension, and nucleus pulposus hernias. Very limited patient knowledge about the disease is evident from patients and family who have never been controlled or treated. Education and intervention are given to patients and families about stroke and its relationship with hypercholesterolemia and a history of hypertension, its complications in the form of pressure ulcers, hernia nucleus pulposus suffered by the patient and its treatment in 3 home visits. In the evaluation found an increase in patient compliance to avoid risk factors and family knowledge about patient care. Unhealthy lifestyles and lack of knowledge of patients about the disease become risk factors for the occurrence of disease in patients. Services with a family medicine approach in pharmacological and non-pharmacological therapy are able to solve health problems and improve the quality of life of patients.

Abstract

Hypertension is one of the non-infectious diseases which becomes an important health problem throughout the world because it keeps increasing. There are 9.4 million deathhs from cardiovascular disease are caused by complications of hypertention. Hypertension itself is a multifactorial disease that arises mainly because of interactions between certain risk factors that can be modified or cannot be modified. Whereas, obesity is caused by an imbalance between the incoming energy and the energy that comes out. The prevalence of obesity throughout the world has tendency to increase in past 30 years.. In addition, Farmingham study stated that 65% of women and 78% of men who suffer from hypertension are directly related to weight gain and obesity. The study was Case Report Study. Primary data is obtained through history taking and physical examination by making home visits, filling out family folders, and filling in patient files. Assessment is carried out based on the initial holistic diagnosis, process, and end of the visit quantitatively and qualitatively. Based on a holistic diagnosis, the patient has a complaint of feeling headache and often followed by a feeling of heaviness behind the neck. The patient also had a history of stroke four years ago. Patient feel worried that the disease is getting worse, has recurrent stroke, and be a burdensome for his family. The patient's expectation is less clinical symptoms , controlled hypertension, no longer obesity, and can do activity and work normally again. Patient was diagnosed with hypertension and obesity in grade II based on the results of history and physical examination. The patient is a 53-year-old man who is obese, often eating foods that contain lots of salt, oily food, and lack of physical activity. Patients are less aware of the disease and the right lifestyle for the illness. After the intervention is done holistically with drugs and education, he obtains a decrease in clinical symptoms and behavioral changes by changing lifestyle and routine taking drugs. Diagnosis and management of this patient has been doing holistically, patients center, family approach based on several theories and current research. In the process of behavior change, patient has reached the trial stage.

Abstract

Thyroid Eye Disease (TED) is a disease of the eye that is characterized by bilateral proptosis which is added by increasing levels of T3, T4 and increasing TSH levels. TED patients are common exposure to keratitis in the cornea that is not properly closed and not completely closed by the palpebra. Corneal ulcers can occur due to keratitis. TED patients can also be approved with glaucoma. 31-year-old man who came with the main complaint that refused perfect vision since 2 weeks of SMRS. The initial complaint of the patient was raised casually for the second time since 3 weeks of SMRS. The patient has a view about the use leaf extract of the right eye so that make red eye and vision decrease increasingly then left eye and enlarge white spots. In patients complaining of weight loss of 14 kg for 1 month released with weakness, nausea, sweating, and shaking compilation in resting conditions. At the time of physical examination found a general situation, the level of compost mentis awareness, blood pressure 140/100 mmhg, pulse 130 x / min. The results of physical examination on the eyes obtained sharp vision of VOD 1/300 and VOS 6/60, exoftamus (+) ODS, ciliary injection (+) OD, conjunctival injection (+) ODS, corneal edema (+) OD, cumhipopion (+ ) OD size 2 mm x 1 mm, shallow anterio OD occer camera, and ODS secret (+). Laboratory examination T3 and T4 increased and TSH decreased. Treatment involves education, antibiotik, antihistamin, kalium, beta blocker, antifungal, diuretic, and steroid.
