Reported a case of female patient 45 years old with diagnose Diabetes Mellitus Type 2 withDiabetic Foot Ulcer Wagner grade 2 at region pedis dextra. Debridement was done in thispatient. The type of debridement was surgical debridement. The purpose of this procedure is toremove necrotic tissue and hyperkeratosis until reach the health tissue. After debridement, thewound was dressed with kassa sterile and elastic bandage. During treatment, there were no pus,bleeding, and unpleasant odor on wound, granulation tissue begin to appear, and pain on thepedis was reduced.

Superficial cervical plexus block is one of the regional anesthesia in neck were limited to thesuperficial fascia. Anesthesia is used to relieve pain caused either during or after the surgery iscompleted. This technique can be done by landmark or with ultrasound guiding. The midpointof posterior border of the Sternocleidomastoid was identified and the prosedure done on thatplace or on the level of cartilage cricoid.

Dermatophytosis is a superficial fungal infection on skin, hair, and nail which caused bydermatophyt. Onicomycosis refers to nail infection which caused by fungal dermatophyt,fungal nondermatophyt, or yeast. Dermatophyt that infected the nail called tinea unguium.Diagnosis for tinea unguium based on the microscopic test with KOH 20%, cultureSaboraud’s dextrose agar (SDA) and histopathologic. In this case reported male, 28 years oldwith changed of the color of nail since four years ago. Dermatologic states is located on tenhand’s fingers and ten foot’s fingers which looked like the nail is thickening, there are keratindebris on the distal part of nail with rough end and nail plate looked like picked up. KOH20% test from nail’s scratch founded long hypha and branched. This patient gives treatmentwith Itraconazol dose 200 mg twice a day for one week in one month and repeated for threemonths. The prognosis is good.

Iron deficiency anemia is a decreament of iron level in the body. Iron deficiency anemiais oftenly seen, especially in the tropical countries or the third world country thatassociated with social economic rate. Iron deficiency anemia happens in more than onethird world’s population. Iron deficiency anemia can be caused by chronic hemorrhage,low intake of iron, absorption disturbance, and increasement of demand. To diagnose irondeficiency anemia, laboratoric examination is needed. The treatment can be causaltreatment, iron supplementation and another additional treatment.

Graves’ disease is the most common form of thyrotoxicosis which caused byautoimmune that attacks the thyroid gland. This disease will lead such followingsyndromes: thyrotoxicosis, goiter, ophthalmopathy, dermopathy. The sensitive TSHassay is the single best screening test for hyperthyroidism, and in most outpatientclinical situations. But when there is a doubt to distinguish diffuse toxic goiter (Graves’disease) from autonomous toxic nodule (Plummer’s disease), radioinuclide scanning canbe performed. I-123 is one of the preferable radiosisotope because it can produce clearimage and short enough half-life, although the price is expensive.

Induction of labor refers to the process whereby uterine contractions are initiated bymedical or surgical means before the onset of spontaneous labor. Several studies haveshown that continuous intravenous infusion of oxytocin is less efficient, particularlywhen there are unfavorable cervical conditions, leading frequently to a cesarean section,because of induction failure. Misoprostol is a cervical modifying agent and laborinductor. The typical effect of a single dose of oral misoprostol is an increase in uterinetonus. Sustained plasma level of misoprostol is required for the development of regularcontractions appear. The bioavailability of vaginal misoprostol also greater compared tooral, sublingual and rectal administration. Using misoprostol for cervical ripening andlabor induction represented a 47% reduction in the risk of having a cesarean section(risk ratio = 0.53). In cases of unfavorable cervix condition, the use of misoprostolcould produce several beneficial effects compared to other inductor such as oxytocin

Fungi disease on skin is often occur at Indonesia because it is a tropic country that has hot climate and also correlate wit bad hygiene. Dermatofitosis is fungi disease on the keratinizing tissue and differentiate as many class such as Tinea Corporis or TineaCruris. Tinea fascialis include in this form. The diagnose this disease from anamnesis,clinical manifetation, and also test result from the tissue. Management for thistineacomprise to topical and systemic. The important one in management this disease isprevention management include in control of skin dryness.

Alopecia areata is hair loss with patchy formation, the most common cause of alopecia nonscarring. Occurred in 1,7 % of Americans aged 50 years. Canoccurs in both sexes, all races and any age. Genetic and immunological factorsplay an important role as a cause of Alopecia areata. The clinical features alopecia areata are round or oval lesions, total baldness, smoothness on the scalpor other parts of the body that has hair. Minoxidil is one of the effective therapyfor Alopecia areata. Known for more than 30 years of minoxidil to stimulate hairgrowth. Minoxidil works on hair follicles, opening the potassium channels, andhave vascular effects that can increase blood flow to hair. Histological studiesshowed that minoxidil therapy may increase the proportion hair follicles in anagenphase and decrease hair follicles at telogen phase. Minoxidil through sulphatmetabolites can open potassium channels, the opening potassium channels canincrease the hair follicles growth. The study of the effects minoxidil on humanepidermal keratinocytes and hair follicles with different culture conditions andmarkers proliferation, found that minoxidil with micro molar concentration canstimulate proliferation both type of cells and all culture condition, whereasminoxidil with milimolar concentration will inhibit cell growth. Typhoid fever is one of the endemic diseases that commonly occur in rural and urban communities. It spreads through food and drink contaminated bySalmonella typhi bacteria. In 2008, detection of typhoid fever was carried out usingthe tubex test in which it principally detects the IgM antibody that specific forSalmonella typhi O9 present in serum by employing the method of InhibitionMagnetic Binding Immunoassay (IMBI). The present study was aimed to evaluateresults of blood sample examination of suspected typhoid fever patient using thetubex test, in order to confirm the typhoid fever diagnose. This research wasretrospective study by analyzing 1.266 data of suspected patients obtained from NikkiMedika Clinic Laboratory. The samples were delivered to the Clinic Laboratory forlaboratory examination since 2008 till October 2012. Each sample was examinedusing the tubex test. The results showed that 11.6 – 27.8% of the blood samples werepositive for typhoid fever while 70 – 88.7% was negative.

Cholera is a type of watery diarrhea with specific sign stool containing mucus which resembles rice water. Cholera caused by gram negative bacteria Vibrio cholerae (V.Cholerae). The transmissions of bacteria were through a contaminated food or water.Bali is an international tourism destination with tropical weather where ice is widelyused in food and beverage which bring a risk of cholera through a contaminated ice.Iceshave a risk of bacterial contamination whether from the making and the usage process.Type of ice that widely used were cube and tube ice which each of them have a differentin making and usage process. The purpose of this study is to obtain the contamination ofV.cholera in cube and tube ice. The method of this study is descriptive observationalstudy with quota sampling technique. Sample were obtained from a restaurants andstreet vendor which use a block and tube ice with total 10 sample and 5 for each type ofice.Sample then cultured in Alkaline Peptone Water(APW) and Thiosulfate Citrate Bilesalt Sucrose(TCBS) agar. Bacteriacolony then identified using a gram staining andLatex Serotyping. The result are 3 over 5 (60%) sample of cube ice contaminated byV.cholera O1 Inaba serotype and 3 over 5 (60%) sample of tube ice contaminated byV.cholera O1 Inaba serotype.
