Abstract

Abstract

Background: Carcinoma of unknown primary (CUP) is a challenging clinical diagnosis, representing histologically confirmed metastatic cancer where the primary tumor site remains unidentified after a standard diagnostic workup. While 18F-FDG PET/CT is recommended by current guidelines, its accessibility is limited in many regions. This report explores the diagnostic utility of Technetium-99m (99mTc)-Sestamibi Single Photon Emission Computed Tomography/Computed Tomography (SPECT/CT) as a valuable alternative. Case presentation: A 67-year-old female presented with musculoskeletal pain. A biopsy of a right ulnar lesion confirmed metastatic carcinoma. Immunohistochemistry (IHC) suggested a possible primary from the lung, breast, or upper gastrointestinal tract. Due to the unavailability of 18F-FDG PET/CT, a 99mTc-Sestamibi whole-body scan with SPECT/CT was performed. The scan identified a metabolically active, malignant-appearing nodule in segment 6 of the left lung and confirmed widespread skeletal metastases. Although a lung biopsy was not feasible due to limited access, the patient was treated with a lung cancer protocol. This resulted in significant clinical improvement. Conclusion: 99mTc-Sestamibi SPECT/CT served as a critical diagnostic tool in this case of CUP. It successfully identified a suspected pulmonary primary, enabling targeted therapy and leading to a positive clinical outcome. This case highlights the modality's efficacy as a tumor-seeking agent and underscores its essential role in the diagnostic armamentarium for CUP, particularly in resource-limited healthcare settings.

Abstract

The combination of online and offline learning had an impact on students' affective knowledge and skills. This study investigated the relationship between combined offline-online learning method and students’ competency, academic grades, and satisfaction in the Childbirth and Newborn Care Course. This study employed a retrospective cohort design and involved 175 students who completed the Childbirth and Newborn Care Course (attending more than 14 sessions) and the final exam. The data collection utilized the lecturer’s grade records and for the evaluation of students’ satisfaction evaluations the existing academic system was used. Chi-square analysis revealed a significant relationship between the combined offline-online method and students’ skill competency (r = 0.000) and academic grades in the Childbirth and Newborn Care Course. However, no significant relationship was observed between the combined offline and online method and student satisfaction. These findings suggested a need for developing teaching method from conventional to combined offline and online learning. The combined offline-online learning must be well-designed and aligned with learning objectives so that students can be engaged in the learning process properly.

Abstract

Postpartum depression is a serious issue affecting mothers after childbirth and can harm both maternal mental health and child development. Hormonal declines postpartum increase vulnerability, often compounded by high stress and limited social support—especially from husbands. This study reviews nine articles from the past five years to examine how husband support maintains postpartum maternal mental health. Database searches with strict inclusion criteria and mixed-method analyses were conducted. Findings show that strong husband support—physical, emotional, and informational—reduces postpartum depression risk by more than twofold. Spousal relationship quality, maternal age, and socioeconomic conditions also influence outcomes, while insufficient husband support can affect fathers’ mental health. This study underscores the need for educating and engaging husbands during the postpartum period. These findings inform inclusive maternal mental health policies and programs, including: adoption and enforcement of routine EPDS screening as standard care (aligned with WHO recommendations and Indonesia’s Permenkes No. 21/2021 on maternal mental health services); strengthening family-centered services that actively involve husbands (partner-inclusive antenatal/postnatal classes, structured father education, couple counseling); culturally sensitive supports across socioeconomic contexts; and clear referral pathways from primary care to specialized services to ensure timely, equitable treatment for all mothers and families.
