Abstract

In the activities of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi, a health check was carried out by a health center health worker, but there was no form of a toddler's medical record that could be used to document the results of a health examination. The target of this community service activity is the Posyandu Kutilang posyandu participants. Guidance and consultation was carried out by Lecturers and Students of the Department of Medical Records and Health Information at the Indonusa Polytechnic Surakarta when accompanying cadres of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency in documenting the health history of toddler participants. The assistance provided includes how to fill out medical record forms for toddlers, what items must be filled in, and how to manage forms. Health checks are only recorded in the agenda book for elderly examinations belonging to the Mobile Puskesmas, so that the participants of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi cannot monitor the results of monthly health checks. Results of Assistance in Making Form Design and Recording Medical Records for Toddlers at Posyandu Kutilang, in the hall of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency was attended by 11 people. Activities started according to the agreed schedule. The training participants participated in the activity enthusiastically because they felt that this activity had a role in administrative order through the implementation of recording in the form / resume of toddler medical records.

Abstract

Kualitas rekam medis dipengaruhi oleh kelengkapan pengisian. Standar kelengkapan pengisian rekam medis setelah selesai pelayanan adalah 100%. Studi pendahuluan didapatkan persentase ketidaklengkapan formulir transfer pasien antar ruangan 74,19% bulan Januari 2023 dan 90,32% bulan Februari 2023. Ketidaklengkapan formulir transfer pasien akan berdampak pada kualitas pelayanan, keselamatan pasien di rumah sakit, capaian laporan analisis rekam medis, akreditasi rumah sakit khususnya dalam elemen penilaian AKP 4. Penelitian bertujuan menganalisis persentase kelengkapan formulir transfer pasien antar ruangan dari Instalasi Gawat Darurat ke Wisma Intensif guna pemenuhan elemen penilaian standar AKP 4 STARKES 2022 dan menganalisis faktor ketidaklengkapan khususnya faktor manusia. Jenis penelitian kualitatif dengan pendekatan deskriptif. Langkah awal analisis kuantitatif lembar transfer dengan cek list hasilnya rata-rata persentase. Selanjutnya dilakukan wawancara mendalam responden. Hasil penelitian persentase kelengkapan formulir transfer pasien sebesar 89,67%. Keterlibatan manajemen belum efektif dalam menumbuhkan kedisiplinan PPA. Pengetahuan PPA baik, namun masih ditemukan ketidaksesuaian antara SPM internal dan SPM nasional terkait waktu pengisian rekam medis rawat inap serta belum mengetahui dengan program kerja dari atasan langsung. Perlu komitmen dari manajemen dan PPA dalam memperbaiki mutu kelengkapan rekam medis dan usulan perbaikan SPM. Peningkatkan kedisiplinan melalui full Electronic Medical Records. Pengembangan penelitian selanjutnya analisis kuantitatif dan analisis kualitatif rekam medis.

Abstract

Recording the results of community health screening is still mostly carried out manually, which often leads to errors and hinders data analysis and reporting. This community service activity aims to introduce and implement a computer-based recording application to support the digitalization of health screening data. The methods used included community education, basic health screening such as measuring height and blood pressure, and recording the results into a simple application developed using data processing software. The results of the activity showed increased recording speed, easier data recap, and more accurate information processing compared to manual recording. The community also gained a better understanding of the importance of regular health screening and the benefits of using information technology. In conclusion, the implementation of a computer-based recording application can improve the effectiveness of health screening data management and strengthen the role of information technology in community health services.

Abstract

Tuberculosis (TB) remains a public health challenge in Indonesia with an incidence of 391 per 100,000 population. Kelurahan Serangan Sub-Health Center (Pustu) lacks standardized TB service flow, causing practice variations and suboptimal coordination. This activity aims to improve suspected TB case service quality through evidence-based service flow socialization and standardization. Methods included existing condition analysis, SOP development, socialization and training, guided implementation, and evaluation. Results showed 100% of health workers (6 people) participated in training with 90% satisfaction rate. SOP implementation achieved 85% adherence in daily practice. There was a 45% increase in TB service practice standardization and 35% reduction in inter-staff variations in case management. Coordination with referral facilities improved by 40% through structured communication system implementation. This activity successfully standardized TB service flow and enhanced health worker capacity in TB early detection at primary care level.
