Abstract

Background: Dementia is a clinical syndrome characterized by progressive loss of cognitive function in elderly people interferes  the ability to function independently. The number of elderly patients increased so fast in worldwide. The prevalence of dementia rapidly increases due to increasing of population. Aging demographic transition is proceeding rapidly especially in China, India, and Latin America, where dementia is rapidly becoming the major public health problem. Demographic data of elderly patients of dementia in Indonesia is still a little. Dementia’s research data in Indonesia were only obtained  from Bali and Yogyakarta, while data from Surabaya were not obtained. Our research data is taken from Dr. General Hospital Soetomo Surabaya, East Java, Indonesia. This study is important for dementia epidemiological data and baseline data for further treatment of dementia patients in Surabaya.Objective: The purpose of this study to determine the characteristics profile of elderly patients  with dementia in Dr. Soetomo General Hospital Surabaya, East Java, Indonesia.Methods: Sampling was done with purposive sampling by observing the medical records of patients in the period of January 2017 to December 2017 based on inclusion and exclusion criterias that have been set. The data was taken from the patient's medical record which had been written and examined by neurologists from the neurology department in the neurobehaviour sub-department of Dr. Soetomo Surabaya.Results: The majority of dementia patients are men (51,7%); the age group which is most at risk for dementia is between age of  75-79 years; most of the patients are jobless; more than half of patients are high school graduates; and Vascular Dementia (VaD) is the type of dementia that  most people suferredConclusion: This characteristic’s data of dementia can be used for providing information about dementia to make public health care provides a better facillities and treatment for elderly patients to prevent dementia burden.

Abstract

Background: Trigeminal Neuralgia is a common condition of facial pain and its significantly affect patients’ daily life. Microvascular decompression is one of the interventional pain management for trigeminal neuralgia. There is still a little data obtained on evaluation of facial pain after microvascular decompression in Indonesia.   Objective: This research aimed to evaluate facial pain after microvascular decompression of  trigeminal neuralgia patients in Dr. Soetomo General Hospital, PHC Hospital, and Bangil General Hospital, Indonesia.Methods: The research design was a pretest-posttest with total sampling. Data were obtained from medical records from January 2018 until June 2019. Researches used Numeric Rating Scale (NRS) as pain measurement. The data obtained were analyzed by descriptive statistical test, normality test, and paired t-test.Results: Trigeminal Neuralgia patients that has been treated with microvascular decompression have an average facial reduction from 7.33±2.29 to 1.89±3.41 with p = 0.001. This result showed that the microvascular decompression has significantly reduce facial pain in patients with trigeminal neuralgia.Conclusion: Microvascular decompression significantly reduce the facial pain of trigeminal neuralgia patients.

Abstract

A single Memory impairment substantially reduces the quality of life in the elderly. It is associated with thealteration of neurotrophic (NT) factors, such as brain-derived neurotrophic factor (BDNF) and glutamatereceptor N-methyl-D-aspartate (NMDA). Exercise is often used to reduce cognitive impairment. Previousstudies show that the benefits of aerobic exercises on such impairments are correlated with increasing BDNFand preventing the production of NMDA. However, some results remain controversial. Thus, the associationbetween exercise and Memory was addressed by examining increases in BDNF and the reduction of NMDAin ageing rats. The study used a randomized, post-test-only controlled group of 30 male one-year-old ageingRattus norvegicus divided into three groups, namely, K0 (control) and K1 and K2 (aerobic swimmingexercise). K1 and K2 animals differed in the frequency of exercise, which is three and four sessions perweek, respectively. Memory was assessed using Y-maze performance. BDNF and NMDA were analyzedusing enzyme-linked immunosorbent assays. A significant improvement in memory function and reductionin the NMDA level were observed in K1 and K2 group rats (p = 0.001; p = 0.041). No significant impact onthe BDNF levels was observed (p = 0.387). Swimming may boost Memory by reducing the NMDA levelbut not by increasing BDNF. Swimming is a promising method for preventing or delaying memory loss indegenerative brain diseases. Further investigation is needed to fully understand underlying mechanisms.

Abstract

Piriformis syndrome (PS) has been documented as a contributory cause for sciatica, buttock, and low back pain. The worldwide incidence of PS would be about 2,4 million per year. The management of piriformis syndrome includes injection of the piriformis muscle with local anesthetic and steroid, however, the study for the effectiveness of the combination is lacking especially in Indonesian population. This case illustrates the effectiveness reduction of triamcinolone in combination with lidocaine measured by Visual Analogue Scale (VAS) in patient suffered from PS.

Abstract

OA adalah penyakit sendi degeneratif kronik progresif yang umum dan menyebabkan gangguan mobilitas sendi sehingga membatasi aktivitas sehari-hari. Hiperglikemia kronik pada DM diduga dapat meningkatkan peradangan sendi dan degradasi kartilago yang meningkatkan risiko kejadian OA dan progresivitasnya. Hasil observasi pada pasien OA lutut di unit rawat jalan saraf RSUD Dr Soetomo Surabaya menunjukkan pasien OA lutut dengan intensitas berat lebih banyak pada penderita DM.Osteoarthritis (OA) of the knee is a common degenerative joint disease which limits daily activities. Many studies suggest a significant relationship between diabetes mellitus (DM) and OA. Chronic hyperglycemia in DM is suspected to increase joint inflammation and cartilage degradation which increases the risk of OA events and their progression. Observations in the neurological outpatient unit of Dr Soetomo Public Hospital Surabaya showed that worse knee OA were more likely to develop among DM patients.

Abstract

Objective: This study aims to investigate the effect of alprazolam administration on sperm viability of mice (Mus musculus) strain Balb/C following CUMSfor 53 days. Method: This study used a true experimental design through randomization. Control group kept undisturbed without treatment and therapy throughout 60 days. K1 and K2 were exposed to a 53 day period of CUMS along with 7-day pretreatment adaptation. K2 was adapted for a week, stressed for 53 days along with cotreatment of alprazolam (4 mg/kg BW). Results: K1 and K2 showed insignificant improvement of sperm viability (p > 0.05).Meanwhile, K0 and K1 showed significant value compared to K2 (p<0.05). K2 showed little increase of sperm viability in CUMS group along with subsequent treatment of alprazolam. Conclusion: Overall, the results indicate an insignificant effect of alprazolam on sperm viability of CUMS-induced mice.

Abstract

Background: There was 90% of Lower back pain (LBP) are non-specific. Acetaminophen is the first an analgesic in the treatment of LBP, but some cases of chronic LBP need additional antidepressant drugs. Amitriptyline is an antidepressant drug often used in cases of pain, but the evidence research in the case of non-specific chronic LBP is still a contradiction. Method: The study was a Double-Blind Randomized Controlled Trial using consecutive sampling admissions. The subjects were divided into 2 groups; acetaminophen plus amitriptyline group and acetaminophen plus placebo group. Results: There was no significant difference in pain intensity of acetaminophen plus amitriptyline and acetaminophen plus placebo groups either statistically or clinically (p = 0.498; OR = 0.667; CI 95% 0.20-2.16; ARR = -0.07 or -7%). The significant improvement of pain intensity in the treatment group Acetaminophen plus Amitriptyline was 24 (72.7%), while in the control group Acetaminophen plus Placebo was 24 (80.0%). In the treatment group was 9 (27.3%) who did not experience a significant improvement in pain intensity, and in the control group was 6 (20.0%). Conclusion: There was no difference in efficacy between acetaminophen plus amitriptyline with acetaminophen plus placebo to reduce pain intensity in non-specific chronic LBP

Abstract

Introduction: Carpal tunnel syndrome (CTS) is a group of neuropathic symptoms regarding to the compression of median nerve which passing through carpal tunnel. There has been a great number of prevalence of CTS in Indonesia, which leads to decreasing quality of life, lack of work productivity, and increasing health cost. Analgesic treatments have been drug of choice for carpal tunnel syndrome for years. However, the effectiveness of the drug and the risk of adverse effect of drugs have always been an issue for analgesic use. An observational study on profile of analgesic drugs administration for carpal tunnel syndrome patients in Dr. Soetomo General Hospital SurabayaMethod: A descriptive observational retrospective study has been conducted to observe the profile of analgesic drugs administration, including type and dosage of drugs, classification of drugs, drugs administration route, early and advanced type of analgesics, and duration of analgesic administration. Sociodemographic data and clinical characteristics (main symptoms) of carpal tunnel syndrome patients are also included in this study.Results: Out of 202 subjects of this study, most patients are women (84,16%), the group age of 50-59 to years old, and the most frequent job is household wife (43,56%). The most common analgesic drugs used for carpal tunnel syndrome patients is 50 mg sodium diclofenac for 78 patients (38.61%). All of those subjects are administered with oral analgesic (100%). 185 patients (91.59%) are administered with analgesic combinations. The duration of analgesic usage are 7 days as an early analgesic in 82 patients (40.59%).Conclusion: CTS is a syndrome due to median nerve compression of the hand, Women, household wife, and age of 50-59 years old are found to be vulnerable to this syndrome. Analgesic drugs mostly used is 50 mg natrium diclofenac, orally, combined, with the period of 7 days for early medication

Abstract

Deep learning semakin berkembang pesat dan banyak dimanfaatkan dalam berbagai bidang kehidupan. Salah satunya bisa dimanfaatkan untuk klasifikasi image medis penderita covid. Keras adalah salah satu framework deep learning yang paling banyak digunakan. Dalam Keras, terdapat beberapa macam algoritma optimizer. Salah satunya adalah optimizer Adam. Untuk menggunakan optimizer Adam ini, perlu menentukan angka learning rate. Penentuan angka learning rate sangat penting karena salah dalam menentukan angka learning rate akan berdampak pada hasil deep learning yang dilakukan. Batch size juga salah satu hyperparameter penting dalam deep learning. Penelitian ini bertujuan untuk mengetahui dan membandingkan beberapa learning rate dan batch size agar diketahui efek dan dampaknya pada hasil loss dan akurasi training dan validasi pada proses deep learning yang dilakukan. Ada 6 learning rate dan 3 batch size yang akan dibandingkan. Hasil yang optimal diantara 6 learning rate dalam penelitian ini adalah 0.0001 dan 0.00001. Sedangkan batch size yang paling bagus hasilnya dari tiga angka yang dibandingkan adalah batch size 5
