Abstract

Hemangioma of the gastrointestinal tract is an uncommon benign vascular tumor. The unspesific clinical sign makes it hard to recognize, thus the clinicians often misdiagnose the disease. This article describes a case of a 2‐years‐old girl who developed recurrent abdominal pain, chronic anemia, and mild abdominal distention that’s tender with a hardness on palpation in the suprapubic region and the inability to urinate.  Laboratory results shows low hemoglobin level.Abdominal ultrasound examination revealed a Douglas pouch abscess or ovarian cyst. The CT scan impression indicates a cystic mass in the Douglas pouch that is compressing the rectosigmoid, possibly indicating an ovarian cyst. Although a diagnosis of sigmoid colon hemangioma is rare and presents itself with non-specific symptoms, this condition should be considered in the differential diagnosis of abdominal tumors in children.

Abstract

Schizencephaly is a rare congenital cerebral malformation within the category of neurological migration defects. Hemiparesis, developmental delays, and seizures are clinical signs that are influenced by both the size and the location of the lesion. This condition is characterized by a cleft that traverses the brain's parenchyma lined by grey-mater (transmantle cleft). In less severe cases, the cleft does not extend into the lateral ventricle. Identification of the cleft's path, accurate assessment of the cortex, and recognition of polymicrogyria are crucial for the radiological evaluation of schizencephaly. We report a case of 18-year-old male diagnosed with closed-lip schizencephaly, which supported by MRI findings. The transmantle cleft and polymicrogyria are most effectively visualized in T2-weighted and FLAIR images. The utility of diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) sequences as well as susceptibility-weighted imaging (SWI) sequence in the context of schizencephaly currently remains uncertain, however it may be useful for ruling out other abnormalities such infarct, hemorrhagic, tumors or other vascular abnormalities. In this context, we emphasize the significance of MRI examination for diagnosing this condition and for recognizing the related abnormalities.

Abstract

Multifocal osteosarcoma is a rare form osteosarcoma. It is diagnosed by having two or more lesions without any visceral damage. Osteosarcoma is characterized by having concurrent numerous lesions; the synchronous type,  and metachronous type are characterized by non-concurrent multiple lesions form. This case reports a boy 4 years old with a left femur osteosarcoma; histopathologically confirmed as chondroblastic osteosarcoma. After 5 months of chemotherapy, a new lesion was found at the left side of the cruris and pedis bones that were multiple, destructive, permeative, and exhibiting periosteal reaction. No visceral damage was found.  In this case, the patient had a primary lesion and new non-concurrently lesions and no visceral damage, as conclusion was diagnosed as metachronous multifocal osteosarcoma that had never been reported in Indonesia.

Abstract

Background The need for CT Scan radiology examinations, which offer more detailed image results than X-rays, continues to increase. Head CT scans are the most frequently performed examinations, reaching 60%. Objective This study aims to evaluate the effective dose distribution of radiation to the tissues involved in head CT-Scan examinations at RS Hermina Bitung and assess its compliance with the regulations set by the Indonesia Diagnostic Reference Level (I-DRL). Methods This study involved 100 patients, consisting of 50 men and 50 women. Data in the form of Dose Length Product (DLP) values were collected from each patient and converted to effective dose using a radiation dose calculator. Results The results showed that 6 out of 100 samples exceeded the dose limit set by the Nuclear Energy Regulatory Agency (BAPETEN). Factors that affect the effective dose include exposure, body weight, object thickness, scan length, age, tissue weight, tissue sensitivity, FOV, and pitch. Nonetheless, the overall average effective dose of the patients was still within the I-DRL. Conclusions Factor such as gender can affect the effective dose received by patients. The 75 percentile value of the effective dose of non-contrast head CT-Scan examination at Hermina Bitung Hospital is in accordance with the provisions of the I-DRL. The radiation dose calculator used can be applied in Indonesia, but still requires increased information literacy regarding its use.
