EVALUASI KERASIONALAN PENGGUNAAN ANTIBIOTIK PADA PASIEN GAGAL GINJAL DI RSUP PROF. DR. R. D. KANDOU MANADO Clara R. Sinaga1), Heedy Tjitrosantoso1), dan Fatimawali1)1)Program studi farmasi FMIPA UNSRAT Manado, 95115  ABSTRACT            The use of antibiotics ratioally could be seen from several parameters, such as right patient, precise indications, proper medication, appropriate dosage, and appropriate delivery time. The use of antibiotics especially in chronic renal failure should be noted because it can cause nephrotoxicity in the kidneys. The use of antibiotics should be considered because some of the antibiotics are toxic to the kidney. This study aims to evaluate the rationally of the use of antibiotics in patient with renal failure in Prof. Dr. R. D. Kandou Hospital, Manado. This research was conducted prospectively during December 2016 – February 2017 at the inpatient installation of Prof. Dr. R. D. Kandou Hospital, Manado. This study was conducted against 40 medical records of patient with kidney failure. The result of this research shows that the evaluation of rationally usage of antibiotics based on exact patients criteria, precise indications, proper medications about 100% , appropriate dosage about 85,71% and appropriate delivery time about 73,62%Keywords : Rationally, Antibiotics, Kidney Failure ABSTRAKPenggunaan antibiotik yang rasional dapat dilihat dari beberapa parameter, yaitu tepat indikasi, tepat pasien, tepat obat, tepat dosis, dan tepat lama pemberian. Penggunaan antibiotik khususnya pada gagal ginjal kronis perlu diperhatikan karena dapat menyebabkan nefrotoksisitas pada ginjal. Penggunaan antibiotik harus dipertimbangkan karena beberapa antibiotik bersifat toksik terhadap ginjal. Penelitian ini bertujuan untuk mengevaluasi kerasionalan penggunaan antibiotik pada pasien gagal ginjal di RSUP Prof. Dr. R. D. Kandou Manado. Penelitian ini dilakukan secara prospektif selama bulan Desember 2016 – Februari 2017 di instalasi rawat inap RSUP Prof. Dr. R. D. Kandou Manado. Penelitian ini dilakukan terhadap 40 catatan rekam medik pasien dengan penyakit gagal ginjal. Hasil penelitian menunjukkan evaluasi penggunaan antibiotik yang rasional berdasarkan kriteria tepat pasien, tepat indikasi, tepat obat sebesar 100%, tepat dosis sebesar 85,71%, dan tepat lama pemberian sebesar 73,62%. Kata kunci: Kerasionalan, Antibiotik, Gagal Ginjal
