Program konversi minyak tanah ke LPG secara resmi diluncurkan oleh Wakil Presiden M. Yusuf Kalla didampingi Menteri Energi dan Sumber Daya Mineral (ESDM) Purnomo Yusgiantoro pada 8 Mei 2007. Sejak itu telah terjadi suatu masalah dalam program ini. Dari awal program diluncurkan telah terjadi ledakan berkali-kali dari penggunaan produk ini. Untuk itu perlu dilakukan penulusuran menggunakan metode RCA agar diperoleh akar penyebab dari ledakan LPG 3 kg yang terjadi selama ini. Penelitian dilakukan di lima tempat kejadian ledakan LPG 3 kg yang semuanya berada di wilayah Semarang. Dalam metode Root Cause Analysis ini dilakukan tahap pengumpulan data, penentuan causal factor, identifikasi root cause, hingga diperoleh rekomendasi dari root cause yang ditemukan tersebut.Root cause yang ditemukan antara lain Kebutuhan pelanggan kurang diperhatikan, Inspeksi produk di bawah standart, pengetahuan konsumen sangat kurang, dan pelatihan dari pemerintah terkait masih di bawah standart. Rekomendasi yang dihasilkan dari root cause yang ditemukan antara lain pelatihan di setiap warung penjual LPG, Pendistribusian yang baik, penyuluhan yang merata, dan inspeksi produk lebih ditingkatkan ABSTRACT Conversion program of  kerosene to LPG was officially launched by Vice President M. Yusuf Kalla was accompanied by Minister of Energy and Mineral Resources (ESDM) Purnomo Yusgiantoro on May 8, 2007. Since then there have been a problem in this program. From the beginning of the program was launched there has been an explosion many times from the use of these products. Therefore we need to research using the RCA method to obtain the root cause of the explosion of LPG 3 kg that occurred during this time. The study was conducted at the scene of the explosion of five LPG 3 kg that are all located in the area of Semarang. In the method of Root Cause Analysis is carried out data collection stage, the determination of causal factors, root cause identification, to obtain recommendations from root cause is found. Root cause found, among others, the customer needs less attention, Inspection products under the standard, consumer knowledge is lacking, and training of relevant government still below standard. Recommendations resulting from the root cause is found, among others, training in every shop selling LPG, good distribution, a uniform extension, and inspection of products is further enhanced.
