Analisa Akar Penyebab Cacat Pada Proses Pengisian Kapsul Jamu dengan Metode Fault Tree Analysis (Studi Kasus PJ. Sabdo Palon)

Perusahaan jamu Sabdo Palon merupakan perusahaan yang  memproduksi obat-obatan tradisional. Proses pengisian kapsul jamu masih dilakukan secara manual. Oleh karena itu kapsul yang dihasilkan seringkali cacat, dalam 1 kali mencetak rata-rata 45% pengisian kapsul  rusak/cacat. Dari masalah tersebut akan dilakukan analisa untuk mengetahui penyebab terjadinya cacat lalu merekomendasikan tindakan pencegahan. Penelitian ini bertujuan untuk menganalisa akar masalah terjadinya pengisian kapsul jamu yang cacat. Gambaran umum permasalahan akan dijelaskan melalui Fishbone kemudian penjelasan akar masalah dijelaskan melalui metode Root Cause Analysis dengan tools Fault Tree Analysis. Setelah menemukan akar masalah, akan diberikan usulan perbaikan untuk mengurangi jumlah produk rusak dengan 5W+1H. Hasil penelitian menujukkan bahwa akar penyebab terjadinya kegagalan produk akhir karena tidak ada spesifikasi khusus dalam pemilihan material luar, bagian penekan pada cetakan yang tidak sesuai dengan bentuk cangkang, bahan cetakan dari plastik sehingga tidak awet, tidak ada prosedur pasti mengenai tata cara pengeringan peralatan dan proses pengisian serta tidak ada catatan produk cacat.   ABSTRACT The titled of this research is Root Cause Analysis of Defect in The Herbal Capsule Filling Process Using Fault Tree Analysis. Sabdo Palon’s herbal company produces traditional medicines. The filling process of herbal capsule  is still manually. Therefore, the resulting capsules are often flawed, in one time averaged 45% capsule filling damaged.  These problems will be analyzed to determine the cause of disability and recommending preventive measures. This study aims to analyze the root causes of the occurrence of the defect herbal capsule filling. General description of the problems will be explained with Fishbone, and then description about  Root Cause Analysis with tools Fault Tree Analysis. And then  will be given the proposed improvements to reduce the number of defective products by 5W + 1H. The results showed that the root cause of the failure because there are no special specifications in material selection is outside, the suppressor on the mold that does not fit with the shell shape, material molding made from plastic so as not durable, there are no procedures for sure about the manner of drying equipment and filling processes and no record of defective products.
