<![CDATA[AbstractMedical records are very important documents in health care, because the contents contained therein illustrate the patient's disease history, actions, treatment and so on ranging from patients coming to health facilities until patients go home. According to the law, complete medical records must be completed before 24 hours after the completion of outpatient services. To avoid the incompleteness of the contents of the outpatient medical record form, a comprehensive study of the medical record form is carried out. This research was conducted at Aisyiyah Padang General Hospital using a descriptive statistical approach. The object of study was the outpatient medical record form in semester one of 2019. The number of samples studied was 99 forms taken at random from a total of 14,958 forms recorded during the semester. From the results of the research conducted, there were 42 forms or 42.4% incomplete forms. The most incompleteness is found in the signature of the doctor handling the patient, which is 38 forms. From these results, it is expected that later the incompleteness of filling out medical records forms for outpatients in Padang Aisyiyah General Hospital can be minimized.Keyword: medical record, outpatient, descriptive statisticsAbstrakRekam medis merupakan dokumen yang sangat penting dalam asuhan kesehatan, karena isi yang terdapat didalamnya menggambarkan riwayat penyakit pasien, tindakan, pengobatan dan lainnya mulai dari pasien datang ke fasilitas kesehatan hingga pasien pulang. Menurut undang-undang, rekam medis yang lengkap harus selesai diisi sebelum 24 jam sesudah selesainya pelayanan rawat jalan. Untuk menghindari ketidaklengkapan isi formulir rekam medis rawat jalan, dilakukan kajian secara konprehensif terhadap formulir rekam medis tersebut. Penelitian ini dilakukan di rumah sakit umum Aisyiyah Padang dengan menggunakan pendekatan statistik deskriptif. Objek yang diteliti adalah formulir rekam medis rawat jalan pada semester satu tahun 2019. Jumlah sampel yang diteliti sebanyak 99 formulir yang diambil secara acak dari total 14.958 formulir yang tercatat pada semester tersebut. Dari hasil penelitian yang dilakukan, terdapat sebanyak 42 formulir atau sebesar 42,4% formulir yang tidak lengkap. Ketidaklengkapan paling banyak terdapat pada bagian tanda tangan dokter yang menangani pasien, yaitu sebanyak 38 formulir. Dari hasil tersebut, diharapkan nantinya ketidaklengkapan pengisian formulir rekam medis pasien rawat jalan di rumah sakit umum Aisyiyah Padang dapat diminimalisir.Kata Kunci: rekam medis, rawat jalan, statistik deskriptif ]]>
