Abstract

AbstractThe medical record alignment system at Yogyakarta Condong Catur Hospital uses the Straight Numerical Filing (SNF) storage system. Yogyakarta Condong Catur Hospital uses the SNF system which often causes misfiles. Therefore, it is necessary to redesign the medical record alignment system. In redesign activities, proactive risk management is needed to prevent failures and minimize risks when redesigning the medical record alignment system using the Failure Mode and Effect Analysis (FMEA) method. This study aims to analyze the risk management redesign of medical record alignment from Straight Numerical Filing (SNF) to Terminal Digit Filing (TDF) with the FMEA method. Design research using action research. An instrument to measure the risk management of the medical record alignment system from SNF to TDF by applying the 8 steps of FMEA. After redesigning, the decline in RPN occurred. The highest decrease in RPN in the storage process of failure mode medical record "officers align the medical records not in the TDF order" with the RPN value decreasing from 336 to 72. Thus, it can be concluded after evaluating and monitoring the implementation of the medical record alignment system redesign from SNF to TDF using the method FMEA is able to significantly reduce the risk in the process of receiving medical record requests, taking and storing medical records.Keyword: Risk management, Redesign, Medical record alignment, Failure Mode and Effect Analysis (FMEA)AbstrakSistem penjajaran rekam medis di RS Condong Catur Yogyakarta menggunakan sistem penyimpanan Straight Numerical Filing (SNF). RS Condong Catur Yogyakarta menggunakan sistem SNF yang sering menyebabkan misfile. Oleh karena itu perlu dilakukan desain ulang (redesign) sistem penjajaran rekam medis. Dalam kegiatan redesign diperlukan suatu manajemen resiko yang bersifat proaktif untuk mencegah terjadinya kegagalan dan meminimalkan resiko pada saat redesign sistem penjajaran rekam medis dengan metode Failure Mode and Effect Analysis (FMEA). Penelitian ini bertujuan menganalisis manajemen resiko redesign penjajaran rekam medis dari Straight Numerical Filing (SNF) menjadi Terminal Digit Filing (TDF) dengan metode FMEA. Design penelitian menggunakan action research. Instrumen untuk mengukur manajemen resiko redesain sistem penjajaran rekam medis dari SNF menjadi TDF dengan menerapkan ke-8 langkah FMEA. Setelah dilakukan redesign terjadi penurunan RPN. Penurunan RPN tertinggi pada proses penyimpanan rekam medis mode kegagalan “petugas menjajarkan rekam medis tidak sesuai urutan TDF” dengan nilai RPN turun dari 336 menjadi 72. Dengan demikian dapat disimpulkan setelah dilakukan evaluasi dan monitoring implementasi redesign sistem penjajaran rekam medis dari SNF menjadi TDF dengan metode FMEA mampu secara signifikan menurunkan resiko pada proses menerima permintaan rekam medis, pengambilan dan penyimpanan rekam medis. Kata Kunci: Manjemen resiko, Redesign, Penjajaran rekam medis, Failure Mode and Effect Analysis (FMEA)
