Literature Review: Faktor-Faktor Internal dan Eksternal yang Mempengaruhi Manajemen Diri pada Pasien Diabetes Melitus

Abstract

Although education has been given, there are still behaviors that are not in accordance with self-management in diabetes. So this study aims to identify internal and external factors that influence self-management in diabetes mellitus patients. The process of selecting articles in this literature review used extensive searches on six databases, namely EBSCO, Scopus, Clinical Key, Science Direct, PubMed, and SAGE publication to obtain quantitative research reports between 2016 and 2020. Stages of selecting articles used the PRISMA model. Of the 529 articles identified, a screening was carried out to obtain 74 articles, then the eligibility which resulted in 11 articles. Identified internal factors were found in 10 of 11 articles including psychological conditions, depression, diabetes distress, experience regarding DM management that was not appropriate, knowledge of DM, low belief in intervention, self-efficacy, medication adherence, motivation, duration of illness, external factors. identified in 4 of 11 articles, namely family and friends (who helped/helpful family and friends, and harmful/harmful family and friends), support from health workers, education, and support roles outside the home (out-of-home informal support). Internal and external factors that exist in patients need to be known in order to help improve self-management in diabetes patients and overcome the obstacles they feel.Keywords: internal factors; external factors; diabetes self managementABSTRAKMeskipun edukasi telah diberikan namun masih terdapat perilaku yang belum sesuai dengan manajemen diri pada diabetes. Maka penelitian ini bertujuan untuk mengidentifikasi faktor-faktor internal dan eksternal yang mempengaruhi manajemen diri pada pasien diabetes melitus. Proses pemilihan artikel pada literature review ini menggunakan pencarian ekstensif pada enam database yaitu EBSCO, Scopus, Clinical Key, Science Direct, PubMed, dan SAGE publication untuk mendapatkan laporan penelitian kuantitatif antara tahun 2016 hingga 2020. Tahapan pemilihan artikel menggunakan model PRISMA. Dari 529 artikel yang diidentifikasi, dilakukan screening sehingga didapatkan 74 artikel, kemudian eligibility yang menghasilkan 11 artikel. Faktor internal yang teridentifikasi terdapat dalam 10 dari 11 artikel diantaranya kondisi psikologis, depresi, distres diabetes, pengalaman mengenai manajemen DM yang belum sesuai, pengetahuan tentang DM, keyakinan yang rendah terhadap intervensi, efikasi diri, kepatuhan pengobatan, motivasi, durasi penyakit. Faktor eksternal yang teridentifikasi dalam 4 dari 11 artikel yaitu keluarga dan teman (yang menolong/helpful family and friends, dan yang membahayakan/harmful family and friends), dukungan tenaga kesehatan, edukasi, dan peran pendukung di luar rumah (out-of-home informal support). Faktor internal dan eksternal yang ada pada diri pasien perlu untuk diketahui agar dapat membantu meningkatkan manajemen diri pada pasien diabetes dan mengatasi hambatan yang dirasakannya.Kata kunci: internal factors; external factors; diabetes self management
