The era that is now developing is the era of the industrial revolution 4.0 which has a rapid increase in technological development. Information technology is also developing rapidly, especially in the health aspect so that it can improve the quality of existing health services. Health services that have high complexity in implementation can be facilitated by the use of management information systems. The implemented management information system can assist health facilities in making fast, efficient, effective and appropriate decisions. The type of research chosen was a descriptive qualitative approach with the aim of knowing the background, challenges, and obstacles in implementing a clinical management information system at the Firdaus 24-hour primary clinic. The data collection method is to conduct observations and interviews with several subjects. Subjects in this study included clinic leaders, programmers, registration admins, service doctors, pharmacists, and cashiers. The results are vision and mission set by the clinic will be a point that must be considered by the clinic and the developer's IT staff from the SIM Clinic. This will make it easier for both parties to develop and implement a Clinical SIM. Based on the implementation component of the Clinical MIS, it was found that human resources acting as IT staff were still minimal in managing and developing Clinical MIS. There has been no training or socialization regarding the features of the Clinical SIM which have undergone development. The facilities and infrastructure in the form of SOPs/regulations, hardware and network have been maximized. However, the software is still not optimal because it is still using the old software version. The Clinic SIM cannot yet be bridging with the SIM owned by BPJS. And have not implemented a billing system on the cashier unit. The conclusion is application of the Clinical SIM at the Firdaus 24-hour Primary Clinic is already good but requires new development to optimize the existing Clinical SIM.
