Abstract

Abstract. CV. Patma Jaya Textile is a textile company focused on making woven sarongs. This woven sarong is intended to meet the needs of society, especially Muslims throughout the world. The company is committed to customer satisfaction and product quality, but faces the challenge of increasing the number of defective products in its woven sarongs. The average defect reached 2.36% in 1 year, exceeding the previous tolerance limit (1.5%). Identification of problems using Fault Tree Analysis shows human, machine, material and environmental factors as causes of defects. The Failure Mode and Effect Analysis method revealed that the largest RPM occurred in potential failure of the surface of the hollow cover, caused by operators who did not follow the company's SOP. Proposed improvements involve increased supervision by the Head of Production, the addition of artificial light at work stations, and training for operators. These steps are expected to reduce defective products. (English) Abstrak. CV. Patma Jaya Textile adalah perusahaan tekstil fokus pada pembuatan sarung tenun. Sarung tenun ini ditujukan untuk memenuhi kebutuhan masyarakat, terutama warga Muslim di seluruh dunia. Perusahaan berkomitmen pada kepuasan pelanggan dan kualitas produk, namun menghadapi tantangan peningkatan jumlah produk cacat pada sarung tenun. Rata-rata cacat mencapai 2,36% dalam 1 tahun, melampaui batas toleransi sebelumnya (1,5%). Identifikasi masalah menggunakan Fault Tree Analysis menunjukkan faktor manusia, mesin, material, dan lingkungan sebagai penyebab kecacatan. Metode Failure Mode and Effect Analysis mengungkap RPM terbesar terjadi pada kegagalan potensial permukaan sarung berlubang, disebabkan oleh operator yang tidak mengikuti SOP perusahaan. Usulan perbaikan melibatkan peningkatan pengawasan oleh Kepala Produksi, penambahan cahaya buatan di stasiun kerja, dan pelatihan untuk operator. Langkah-langkah ini diharapkan dapat mengurangi produk cacat. (Indonesia)
