<![CDATA[Patients with renal impairment have different pharmacokinetic parameters from those with normal renal function. Therefore, with kidney function that is no longer normal, administering appropriate and appropriate drugs to patients with kidney failure is important to avoid adverse drug reactions and achieve optimal drug therapy. This research aims toanalyzed drugs requiring dose adjustment in patients with kidney failure based on creatinine clearance calculations with the Cockroft Gault formula, Chronic Kidney Disease Epidemiology Collaboration (CKD EPI) and Modification of Diet in Renal Disease (MDRD). The study was conducted using a cross-sectional method with prospective data collection by tracing data from the medical records of kidney failure patients for the period January-April 2023. Next, creatinine clearance was calculated and the dose suitability was assessed in reference to Renal Pharmacotherapy 2nd edition in 2021 and The Renal Drug Handbook 5th edition 2019. Data analysis was carried out descriptively and the Chi-Square test with SPSS to see the effect of the suitability of the dose with the formula used. The number of patients who met the inclusion criteria was 68 patients. Of the 213 drugs prescribed, 210 drugs (98.60%) were dosed correctly and 3 drugs (1.40%) were not dosed correctly. Drugs that were not properly dosed were allopurinol, ketorolac injection, and codeine. The results of the Chi-Square showed no significant difference in the suitability of the dose with the Cockroft Gault formula, Chronic Kidney Disease Epidemiology Collaboration (CKD EPI), and Modification of Diet in Renal Disease (MDRD) formulas (p = 1.000).
