The Pratama Clinic of UIN Sunan Ampel Surabaya faces significant challenges in reputation risk and service risk development, particularly in the digital era. This study aims to assess these risks based on ISO 31000 standards. The objectives are: (1) identifying potential reputation and service risks, (2) analyzing the alignment of the clinic’s risk management system with ISO 31000, and (3) recommending strategies to enhance risk management effectiveness. Findings reveal: (1) critical risks, including operational, regulatory compliance, and patient safety risks, (2) partial misalignment of the current risk management system with ISO 31000 principles, and (3) recommendations such as staff training, internal policy enhancement, and technological improvements. Utilizing a qualitative case study with interviews, observations, and document analysis, this study underscores the need for proactive risk management, service quality improvements, and an effective complaint handling system to uphold the clinic’s professional reputation.
