<![CDATA[Premature birth remains a global issue. The transition from hospital care to home care remains a challenge, as premature babies need to adapt to the environment outside the incubator. Premature babies are a major factor contributing to the risk of stunting. This community service programme aims to improve the ability of health workers to provide care to premature babies and to assist family support teams (TPK) in supporting families with premature babies. The method used was participatory integrative continuity of care (CoC). This activity was carried out at the Kedungmundu Community Health Centre in Semarang, from 1 June to 30 August 2025. This activity was conducted through interactive lectures and discussions, participatory training, TPK peer support, and family assistance. The results of the activities showed an increase in the knowledge of health workers in the management of premature babies after hospitalisation, an improvement in the management of TPK assistance, and an increase in the ability of TPK to assist families in caring for premature babies. This activity can serve as a CoC model for all health workers and TPK to implement the In'prematuro system as a natural incubator premature babies.]]>
