Analisis Implementasi Telefarmasi Berbasis Medication Therapy Management pada Pasien Diabetes Melitus

Abstract

Diabetes Melitus (DM) memerlukan penatalaksanaan berkelanjutan melalui Program Pengelolaan Penyakit Kronis (Prolanis). Kolaborasi interprofesional melalui telefarmasi berbasis lima elemen Medication Therapy Management (MTM) diperlukan untuk mengoptimalkan peran aktif pasien dan pemantauan terapi oleh apoteker. Penelitian ini bertujuan untuk menganalisis pengaruh telefarmasi berbasis MTM terhadap Gula Darah Puasa (GDP) dan kejadian Drug-Related Problems (DRP) pada pasien DM. Penelitian ini menggunakan desain quasi-experimental one group pretest-posttest. Subjek penelitian adalah pasien Prolanis DM di Puskesmas Babat periode Desember 2023 – Januari 2024 sesuai kriteria inklusi (n=53). Hasil penelitian menunjukkan penurunan rata-rata GDP dari 245 mg/dL menjadi 213 mg/dL. (nilai p = 0,004). Terjadi penurunan total kejadian DRP dari 53 kasus menjadi 44 kasus. Terdapat penurunan DRP pada kategori P1.2 (efek obat tidak optimal) sebanyak 7 pasien, C7.1 (ketidakpatuhan sengaja) sebanyak 51 pasien, C7.2 (penggunaan obat berlebih) sebanyak 1 pasien, dan C7.10 (kurangnya pemahaman instruksi) sebanyak 10 pasien. Telefarmasi berbasis MTM berpengaruh signifikan dalam menurunkan GDP dan meminimalkan kejadian DRP pada pasien Prolanis DM di Puskesmas Babat. Apoteker dapat mengintegrasikan telefarmasi berbasis MTM sebagai standar operasional prosedur (SOP) dalam pelayanan Prolanis untuk mempertahankan kontrol glikemik pasien. Diabetes Mellitus (DM) requires continuous management through the Chronic Disease Management Program (Prolanis). Interprofessional collaboration via telepharmacy, based on the five elements of Medication Therapy Management (MTM), is essential to optimize active patient participation and therapeutic monitoring by pharmacists. This study aimed to analyze the effect of MTM-based telepharmacy on Fasting Blood Glucose (FBG) and the occurrence of Drug-Related Problems (DRPs) in DM patients. This study employed a quasi-experimental one- group pretest-posttest design. The subjects were 53 Prolanis DM patients at Puskesmas Babat from December 2023 to January 2024 who met the inclusion criteria. The results showed a statistically significant decrease in average FBG from 245 mg/dL to 213 mg/dL (p = 0.004). The total occurrence of DRPs decreased from 53 cases to 44 cases in the following categories: DRP P1.2 (suboptimal drug effect) by 7 patients, C7.1 (intentional non-compliance) by 51 patients, C7.2 (overuse of medication) by 1 patient, and C7.10 (lack of understanding of instructions) by 10 patients. The provision of MTM-based telepharmacy significantly reduces FBG and minimizes the occurrence of DRPs in Prolanis DM patients at Puskesmas Babat. Pharmacists can integrate MTM-based telepharmacy as Standard Operating Procedures in Prolanis to maintain patient glycemic control.
