<![CDATA[The rising number of motor vehicles in Yogyakarta has increased activities at local gas stations, raising occupational health and safety (OHS) risks. The aim of this study is to analyze operator safety risks at XYZ Gas Station in Yogyakarta by integrating Hazard Identification, Risk Assessment, and Risk Control (HIRARC) with Failure Mode and Effect Analysis (FMEA). The research involved three stages: observation to identify hazards, risk level assessment using a HIRARC questionnaire distributed to operators, and mitigation priority analysis calculating the Risk Priority Number (RPN) via FMEA. Initially, 26 hazards were identified across five operator job types. The HIRARC assessment revealed five high-level risks, which were further analyzed through FMEA and managerial interviews. Results highlighted three critical risks with high RPN: operators inhaling dense fuel vapors due to poor SOP supervision (RPN 80), consumers failing to turn off engines due to unseen signs (RPN 64). The study recommends improving markings and signs, enforcing SOP supervision, and implementing the use of technology in occupational health and safety supervision to minimize workplace risks.]]>
