This research is motivated by the importance of implementing the Occupational Safety and Health Management System (SMK3) as an effort to prevent work accidents and occupational diseases in accordance with the provisions of the Minister of Health Regulation concerning the Implementation of the Hospital K3 Management System. This study aims to analyze the level of implementation of SMK3 in hospitals and its compliance with the standards set out in the regulation. The method used is descriptive research with a qualitative approach through observation, interviews, and documentation studies of SMK3 elements including policies, planning, implementation, monitoring, and evaluation. The results of the study indicate that the implementation of SMK3 has been running in most elements, but there are still several aspects that need to be improved, especially in the consistency of documentation, training of health workers, and periodic evaluations. The conclusion of this study is that the implementation of SMK3 based on the Minister of Health Regulation No. 66 of 2016 plays an important role in creating a safe and healthy work environment, and requires management commitment and active participation of all workers so that its implementation can run optimally and sustainably
