Auditory hallucinations are common positive symptoms in patients with schizophrenia, characterized by sensory perceptions occurring without external stimuli. This nursing case study aimed to document and analyze the application of Al-Qur’an murottal audio therapy (Surah Ar-Rahman) in reducing hallucination scores among patients with auditory hallucinations. A descriptive case study design was used involving six respondents diagnosed with paranoid schizophrenia at Yayasan Bagus Mandiri Insani Palembang. The intervention consisted of listening to murottal Surah Ar-Rahman through an MP3 player and earphones for 15 minutes once daily for three consecutive days, from 25 to 27 June 2026. Hallucination levels were assessed using the Auditory Hallucinations Rating Scale (AHRS) before and after each therapy session. Following the three-day intervention, five of the six respondents demonstrated meaningful reductions in total AHRS scores. Improvements were particularly observed in the ability to control voices, as well as reduced loudness, duration, negative content, and pressure of the voices. Patients also reported feeling calmer and more relaxed. Therefore, Al-Qur’an murottal audio therapy (Surah Ar-Rahman) may be considered a complementary nursing intervention to help reduce auditory hallucination symptoms in patients with schizophrenia.
