Abstract

Hypertension is a medical condition characterized by systolic blood pressure ≥140 mmHg and diastolic blood pressure ≥90 mmHg, often referred to as the silent killer. One effective and practical non-pharmacological therapy that can be carried out within the family setting is isometric handgrip exercise. This case study aims to provide an overview of the implementation of isometric handgrip exercise and its effect on blood pressure changes in families with hypertension. The case study used a descriptive method with a family nursing care approach involving two subjects. The intervention was conducted over three consecutive days, with each session lasting 30 minutes (4 sets, 2-minute contractions, 1-minute rest periods). Data were collected through interviews, observations, physical blood pressure measurements, and calculation of Mean Arterial Pressure (MAP). The results showed that Subject I (Mrs. K, 61 years old) experienced a reduction in blood pressure from 150/93 mmHg (MAP 112 mmHg) to 132/84 mmHg (MAP 100 mmHg). Subject II (Mrs. T, 50 years old) experienced a reduction from 158/98 mmHg (MAP 118 mmHg) to 134/84 mmHg (MAP 100 mmHg). These findings indicate that isometric handgrip exercise performed over three days can reduce blood pressure and MAP values in both subjects. This therapy is recommended as a complementary non-pharmacological approach in the family-based management of hypertension.
