Therapeutic communication plays a crucial role in enhancing the quality of nursing care and patient satisfaction, particularly within Indonesia's culturally and linguistically diverse healthcare system. Despite its importance, limited empirical research has been conducted on the impact of therapeutic communication on perceived care quality and patient satisfaction within the BPJS system. This Study to analyze the effect of therapeutic communication on patients' perceptions of nursing care quality and their level of satisfaction among BPJS-insured patients in Indonesia. The methods are a quantitative, cross-sectional descriptive-correlational study was carried out at Wonogiri General Hospital. The sample consisted of 100 nurses and 100 BPJS patients selected through purposive sampling. Data was collected using validated questionnaires: Global Interpersonal Therapeutic Communication Scale (GITCS©), Quality of Nursing Care Scale (QNCS), Quantitative Nursing Practice Tool for Communication (QNPCT), and Patient Satisfaction with Nursing Care and Quality Questionnaire (PSNCQQ). Data analysis involved descriptive statistics, Pearson correlation, regression analysis. The Results of this study are a significant strong positive correlation (r=0.68, p<0.01) was found between therapeutic communication and patient satisfaction. Regression analysis indicated that patient age and educational level significantly predicted perceptions of communication effectiveness (p<0.05), with older patients perceiving communication more favorably and those with higher education levels reporting lower perceptions of its effectiveness. Therapeutic communication significantly influences patients’ perceptions of care quality and satisfaction among BPJS-insured patients. Developing adaptive communication skills tailored to patient demographics is recommended to improve health outcomes and patient experience.
