ABSTRACT Background: Maternal–neonatal blood group incompatibility may increase the risk of red blood cell alloimmunization and hemolytic disease of the newborn. Objective: This activity aimed to improve pregnant women’s knowledge, determine the distribution of ABO and RhD blood groups, and identify alloimmunization risk at Gotong Royong Hospital, Surabaya. Methods: The activity included education, pre-test and post-test assessments, ABO and RhD blood group testing, and interviews regarding obstetric and transfusion histories among 30 pregnant women. Results: The mean knowledge score increased from 58.3±12.1 to 84.7±8.9. Blood group O was the most common, identified in 12 participants (40.0%). Twenty-nine participants (96.7%) were RhD-positive, while one participant (3.3%) was RhD-negative. Six participants (20.0%) had at least one alloimmunization risk factor. Recommendation: Regular education and screening should be continued, followed by red blood cell antibody testing for at-risk pregnant women.
