Association of leukocyte and blood glucose levels with traumatic brain injury severity: A cross-sectional study at Wahidin Sudirohusodo General Hospital

Abstract

Background: Traumatic Brain Injury (TBI) is a functional disorder of the brain due to external trauma that injures the brain with the most common cause being road traffic accidents. The severity of TBI is divided into three categories based on the Glasgow Coma Scale (GCS) score: mild, moderate, and severe. TBI can trigger a systemic stress response that may lead to changes in leukocyte counts and blood glucose levels.Objectives: This study aimed to determine the association between admission leukocyte count, blood glucose levels, and the clinical severity of TBI. Methods: This analytical cross-sectional study utilized secondary data obtained from the medical records of 209 patients with TBI treated at Wahidin Sudirohusodo General Hospital between 2022 and 2023. Data on participants’ demographic and clinical characteristics, including sex, age, patient outcome, cause of injury, and type of trauma were collected. TBI severity was classified according to GCS scores as mild, moderate, or severe. Admission leukocyte counts and random blood glucose levels were also recorded. Statistical analysis employed the Chi-Square test, one-way ANOVA, and Spearman's rank correlation test.Results: A significant positive association was found between elevated leukocyte count and TBI severity (p<0.001), and between elevated admission blood glucose level and TBI severity (p<0.001). The mean leukocyte count and blood glucose level were highest in the severe TBI group (18,217.39 ± 5,910.59 cells/μL and 190.49 ± 92.10 mg/dL, respectively). However, the correlation between leukocyte count and blood glucose level themselves was not statistically significant (r=0.129, p=0.062).Conclusion: Leukocyte and blood glucose level have significant association with severity of TBI. These results suggest that leukocyte and blood glucose levels may function as important parameters in the initial assessment protocol for patients with traumatic brain injury.
