Abstract

Highlights: Notable findings include a substantial decline in the prevalence of adult allergic diseases between 2013 and 2017, attributed to the implementation of the JKN program in Indonesia. The effectiveness of the JKN in shifting allergy care from specialized to primary healthcare settings.   Abstract Introduction: Allergic diseases significantly impact global public health. These conditions, which include allergic asthma, rhinitis, atopic dermatitis, food, drug, and insect sting allergies, lead to a decline in quality of life and increase economic burdens. Despite their high prevalence, detailed epidemiological data on adult allergic diseases, particularly in the Eastern region of Indonesia, are lacking. Methods: This observational, descriptive study used a retrospective cross-sectional design to analyze 465,014 outpatient visits from 2013 to 2017 at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. It focused on adult patients diagnosed with various allergic diseases, examining their characteristics, clinical manifestations, risk factors, and treatment outcomes. Results: This study documented a significant 81.3% decline in the prevalence of adult allergic disease between 2013 and 2017, partly attributable to the implementation of the National Health Insurance (JKN) program in Indonesia. The prevalence among adults was 2.9%, with allergic asthma being most common. A marked gender disparity was observed, with females more frequently affected than males. The findings also underscored the critical role of comorbid conditions in the comprehensive management of these diseases. Conclusion: This study highlighted the profound influence of healthcare reforms on the accessibility and management of allergy care in Indonesia, underscoring the necessity for disease-specific diagnostic, treatment, and management strategies that account for gender and age differences. It also emphasized the importance of integrated care approaches for patients with comorbid conditions and calls for further longitudinal research to explore the dynamics of allergic diseases more deeply.
