Table slide jamming on the Reform HR PSGS-4070AH surface grinding machine can reduce machining quality, disrupt laboratory activities, and increase the risk of component failure. This study aimed to identify the root cause of table slide jamming using the Root Cause Failure Analysis (RCFA) method. Data were collected through observation, interviews, component inspection, and documentation, and subsequently analyzed using the Fishbone Diagram and 5 Why Analysis. The results revealed that the root cause was not the failure of the ball screw, but the absence of a consistent preventive maintenance program, resulting in an ineffective central lubrication system. The proposed corrective actions include restoring the lubrication system, replacing damaged components, and implementing a preventive maintenance program to improve machine reliability.
