Reconceptualizing Field Assessment Stroke Triage for Emergency Destination Decision: A Systematic Literature Review

Abstract

This study aims to reconceptualize field stroke triage as a decision-making process for determining the emergency destination that integrates stroke recognition, severity assessment, estimation of therapeutic needs, supporting technologies, and the readiness of the healthcare system. The study utilized a Systematic Literature Review design with narrative synthesis. The search was conducted in Scopus using the keywords “Stroke AND Triage” for publications from 2020 to 2025. Of the 238 records identified, 40 articles satisfied the inclusion criteria and were analyzed based on study design, triage instruments, transportation strategies, technology, service outcomes, and the risk of mistriage. The results of the synthesis indicate that field assessment occurs as a series of decisions beginning with the emergency call, clinical examination in the field, teleconsultation, route selection, and activation of the destination hospital. Prehospital clinical scales help standardize assessments, but their performance is influenced by the severity of deficits, stroke mimics, time of onset, and regional context. Telemedicine, computerized imaging, and artificial intelligence have the potential to improve accuracy and efficiency; however, evidence regarding their readiness for implementation remains inconsistent. Destination decisions were also influenced by arrival time, availability and suitability of facilities, network capacity, and the balance between overtriage and undertriage. The study concluded that field stroke triage assessment should be understood as a multidimensional, adaptive, probabilistic, and systematic decision making framework.
