<![CDATA[Medical records provide numerous benefits, including support for treatment, research, and legal evidence. Medical Record Officers are responsible for preparing Visum et Repertum, an important medical report required by investigators and judges. The preparation of this report involves both physicians and medical record officers, who must understand the requirements for submitting requests. The quality of the Visum et Repertum preparation varies, and the document must comply with legal provisions. Information technology innovation can improve efficiency and accuracy in the Visum preparation process, minimize errors, and facilitate rapid fulfillment of requests. This study focuses on the development of a web-based electronic system for preparing Visum et Repertum in private hospitals in Semarang City. This study employed a combination of Research and Development (R&D) and descriptive research methods. The Research and Development method was used to produce a specific product and test its effectiveness. Meanwhile, descriptive research aimed to clearly describe social situations or phenomena for application development using the Waterfall Model. User perceptions of the system were then evaluated using the HOT-Fit method. The results showed that the Udinus research team successfully developed the VISMED application using a systematic approach based on user needs, both from medical record management and legal compliance perspectives. The system is capable of facilitating the process of recording requests, preparing Visum et Repertum, delivering Visum results to applicants, and storing request histories. Evaluation of the VISMED system using the HOT-FIT model questionnaire with 30 medical record officer respondents revealed that the Human component scored 5.65, Technology scored 5.40, FIT scored 5.36, and Organization scored 5.05, all categorized as good. Most users considered the application easy to use, with features functioning properly, while the interface design was regarded as sufficiently attractive and supportive of a positive user experience.]]>
