Background: Cancer, including colorectal cancer, poses a significant global health burden and is the third most prevalent malignancy worldwide. Indonesia, with one of the largest populations globally, recorded 35,676 cases of colorectal cancer from 2017 to 2022. Chemotherapy is recommended as a treatment for colorectal cancer in Indonesia's national guidelines. Objective: This study aimed to analyze the chemotherapy regimen and dosing accuracy for stage III and metastatic colorectal cancer. Methods: An observational cross-sectional study was conducted retrospectively among all colorectal cancer chemotherapy visits in a hospital setting from January to June 2024. The chemotherapy profile and dosing accuracy were assessed according to the national guidelines. Results: This study found that FOLFIRI was the most commonly used treatment (71%), followed by FOLFOX (29%). No targeted therapy was used. In stage III, 20% of FOLFOX and 44% of FOLFIRI were administered with dose rounding, while 8% of FOLFOX and 28% of FOLFIRI were administered outside the recommended range. All five cytotoxic drugs were also administered outside the recommended dose rounding. In the metastatic stage, only FOLFOX was given outside the recommended range (11%), specifically 5-FU at 1200mg/m2 IV continuous. Conclusion: The chemotherapy regimens have been in accordance with the national guidelines. The correct dosing was still greater, although some chemotherapy doses were still found outside the recommended dose range.
