The presence of infection at the extraction site can affect the timing of implant placement and the success of treatment. This case report describes the ma-nagement of the loss of the right lower first molar using the early two-stage implant placement technique following a change in the treatment plan. A 25-year-old man presented with a residual root of tooth 46 and wished to undergo rehabilitation using implants. Clinical examinations and CBCT scans were performed for treatment planning. Immediate implant placement was initially planned; however, an abscess was discovered during extraction, leading to a change in the plan to early implant placement. Following an eight-week healing period, the implant was placed using a two-stage surgical technique, followed by the place-ment of a healing abutment and a final restoration in the form of a screw-retained crown. Clinical and radiographic evaluation demonstrated good healing and implant stability. It was concluded that the early two-stage implant placement technique yields good clinical results in areas with a history of local infection when supported by an appropriate treatment plan and an adequate healing period.
