The implementation of Electronic Health Records (EHR) is part of healthcare digital transformation aimed at improving service quality and efficiency. This study evaluates the implementation of the Electronic Medical Record (EMR) system at Mekar Sari Hospital Bekasi using DeLone and McLean’s information system success model. A quantitative descriptive design was applied to 50 EMR users selected through total sampling. Data were collected using a questionnaire covering six dimensions: system quality, information quality, service quality, use, user satisfaction, and net benefits, and analyzed descriptively using mean values and frequency distributions. The results showed that system quality scored 3.48 (good), information quality 3.96 (good), service quality 3.70 (good), use 4.22 (very good), user satisfaction 3.90 (good), and net benefits 4.04 (good). The highest score was found in the use dimension, while system quality received the lowest score but remained in the good category. Overall, the EMR implementation at Mekar Sari Hospital Bekasi was considered successful across all dimensions of the DeLone and McLean model. Continuous evaluation, user competency improvement, and system development are recommended to maintain and enhance these outcomes.
