Overnutrition among adolescents is a potential risk indicator for future metabolic disorders, thus requiring early identification through BMI-for-age (BMI/A) screening. This study aimed to describe the distribution of nutritional status based on BMI/A, measure physical activity levels, and analyze the effect of physical activity on nutritional status among 100 male students (santri) at an Islamic boarding school in Gontor Ponorogo Regency. The research design used an analytical observational study with a cross-sectional approach. Nutritional status was determined using BMI/A according to WHO 2006 through WHO AnthroPlus and categorized based on Ministry of Health Regulation No. 2 of 2020, while physical activity was measured using the Indonesian version of the Physical Activity Questionnaire for Adolescents (PAQ-A) (α = 0.880). Bivariate analysis used the Pearson Chi-Square and Likelihood Ratio tests (α = 0.05). Results showed that the distribution of nutritional status was 6.0% underweight, 72.0% normal, and 22.0% overnutrition. As many as 85.0% of respondents had not achieved sufficient physical activity. Statistical testing showed no significant association between physical activity and nutritional status (p = 0.653; LR p = 0.511), although a descriptive pattern was observed showing an increasing proportion of normal nutritional status alongside higher physical activity (66.7%–80.0%). The 22.0% proportion of overnutrition identifies a metabolic risk that warrants attention. Further research is recommended to integrate direct metabolic examinations and multivariate analysis with control of confounding variables.
