<![CDATA[Various efforts have been made to accelerate the reduction of maternal and infant mortality rates, including ensuring mothers’ access to quality health care, delivery assistance by trained health workers, postpartum and neonatal care, an effective referral system, and family planning services, including postpartum family planning. This case study aims to examine the continuity of care (COC) in midwifery care provided to Mrs. U at the Eva Clinic and Maternity Home in Klurak Village, covering pregnancy, childbirth, the postpartum period, the newborn, and family planning. The methods used in this case were observation and interviews, which began when Ms. U was in her third trimester of pregnancy and continued through her use of contraception. The study involved a case study of one patient who received five midwifery care interventions at the Dan Rb Eva Clinic in Klurak Village, using a continuity-of-care approach. The midwifery care provided included assessment through evaluation from August 10, 2025, to August 27, 2025. Conclusion: Five midwifery care interventions were provided in this case study as part of continuity of care, or comprehensive midwifery care, in accordance with midwifery standards. From the beginning of the assessment to the end, care was provided in accordance with standards, enabling the patient to successfully navigate her pregnancy and transition to physiologically-based family planning. The results obtained from the case study and patient observations showed no discrepancies with the theory.]]>
