IMPLEMENTASI PEMBERIAN POSISI HEAD UP 30 DERAJAT DALAM MENINGKATKAN HEMODINAMIK PADA PASIEN INFARK MIOKARD DI RUANG IGD RS IBNU SINA YW UMI MAKASSAR

Abstract

Acute myocardial infarction is one of the leading causes of death in cardiovascular disease cases. In addition to pharmacological therapy, non-pharmacological nursing interventions play an important role in helping to maintain the patient's hemodynamic stability. This case study aims to assess the effectiveness of applying a 30-degree head-up position in improving hemodynamic conditions in myocardial infarction patients treated in the emergency room at Ibnu Sina YW UMI Hospital in Makassar. The research subject was a 60-year-old man with a medical diagnosis of myocardial infarction accompanied by nursing problems in the form of decreased cardiac output. The interventions provided included monitoring vital signs, oxygen therapy, education on healthy lifestyles, and setting the head up 30 degrees. After 10 minutes of intervention, there was an improvement in hemodynamic parameters, marked by an increase in oxygen saturation from 93% to 98%, blood pressure from 101/46 mmHg to 110/50 mmHg, and an improvement in respiratory rate from 12 breaths/minute to 18 breaths/minute. However, peripheral perfusion did not show optimal improvement, as evidenced by a CRT of more than 3 seconds and decreased level of consciousness. The 30-degree head-up position can be a simple, effective, and safe non-pharmacological measure to improve oxygenation while helping to maintain hemodynamic stability in myocardial infarction patients. Even so, this intervention still needs to be combined with other medical therapies in order to achieve maximum results.
