Product quality is an important factor in the manufacturing industry to maintain customer satisfaction and reduce losses caused by defective products. In the production process of 350 mL Teh Pucuk Harum at PT. Tirta Fresindo Jaya Pasuruan, problems are still found at the after-labeling stage, such as misaligned labels, missing labels, and incorrect label placement, which result in rejected products. This study aims to determine the quality level of the labeling process, calculate the DPMO (Defects Per Million Opportunities) and sigma level, and identify the factors causing product defects. This research uses the Six Sigma method with the DMAIC approach (Define, Measure, Analyze, Improve, Control). Data were collected through direct observation, company documentation, and interviews conducted over six days of observation. The results showed that label rejects were the most dominant type of defect compared with bottle and cap rejects. The analysis was conducted using control charts, DPMO, sigma level, and a cause-and-effect diagram (fishbone diagram) to identify the root causes of defects. The proposed improvements, including the addition of lighting in the sorting area and improvements to the sorting formation, were proven to reduce the number of label rejects by 31.68%, increase production process efficiency, and improve product quality continuously.
