Acute diarrhea remains a common cause of childhood morbidity, and inappropriate antibiotic use is still a concern in primary care. This study aimed to identify demographic and clinical factors associated with antibiotic use in children with acute diarrhea. A cross-sectional study was conducted at Wayabula Primary Health Care Center, Pulau Morotai, involving 53 medical records. Variables included gender, age, and degree of dehydration. Data were analyzed descriptively and using Fisher’s exact test. Most children were male (62.3%) and under five years (83.0%), with antibiotics prescribed in 11.3% of cases. Antibiotic use was not associated with gender or age (p = 1.000) but was significantly associated with dehydration (p < 0.001).
