This study aims to analyze failures in the Fan Extract component P/N CE1909AA1 installed on Airbus A330 aircraft at PT. GMF AeroAsia Tbk. The Fan Extract is a vital part of the avionics ventilation system, preventing overheating of electronic equipment. Engineering data from 2019 to 2024 recorded 66 unscheduled removals, indicating recurring issues requiring further investigation. Pareto analysis shows the main cause of failure is Bearing Rough (47%). Using the Fishbone Diagram with the 5M, root causes of Bearing Rough were identified as non-compliance with procedures, incorrect greasing, lack of cleaning procedures in job cards, uncalibrated tools, low bearing material quality due to OEM supply limitations, and improper aircraft storage conditions causing lubricant contamination. Corrective actions recommended include revising maintenance procedures, calibrating tools, retraining personnel, and improving storage policies. Preventive actions involve condition-based maintenance scheduling and procuring OEM-standard lubricants and tools
