<![CDATA[Background: Perineal pain is a common complaint among postpartum women, particularly following vaginal birth accompanied by perineal trauma. Episiotomy is one form of perineal trauma that may cause tissue injury and affect maternal comfort. Objective: This study aimed to determine the relationship between a history of episiotomy during childbirth and perineal pain intensity among postpartum women. Methods: This analytic observational study used a cross-sectional approach. The sample consisted of 60 postpartum women with vaginal deliveries: 30 with a history of episiotomy and 30 without episiotomy. Pain intensity was assessed using the 0–10 Numeric Rating Scale. Bivariate analysis used the Mann-Whitney U test with α=0.05. Results: The mean pain score was 5.17 ± 1.34 (median 5.5) in the episiotomy group and 3.27 ± 1.26 (median 3.5) in the non-episiotomy group. The Mann-Whitney test showed U=774.5 and p<0.001. Conclusion: A significant relationship was found between a history of episiotomy and perineal pain intensity among postpartum women. ]]>
