The reporting of unsafe conditions and unsafe actions at PT Pelindo Multi Terminal Branch Jamrud Mirah Nilam and TCK Teluk Lamong previously relied on an unintegrated Google Form, resulting in delayed follow-up, scattered and unstructured documentation, handling status that was difficult to track accountably, finding locations that could not be visualized spatially, and reduced employee motivation to report due to the absence of participation incentives. This study aims to design and build a web-based Occupational Health and Safety (OHS) reporting information system that integrates Geographic Information System (GIS)-based location mapping, a points-based leaderboard gamification mechanism, and real-time notifications to accelerate the reporting process and OHS decision-making. The system was developed using the Agile method through iterative sprints, employing React JS as the frontend technology and Firebase Firestore as a real-time NoSQL database consisting of four main collections: users, unsafe_actions, unsafe_conditions, and notifications. Requirements were gathered through field observation, interviews with OHS and IT division staff, and literature study, while system functionality was verified through Black-Box Testing on eleven main usage scenarios. The results show that all eleven test scenarios ran validly, covering user authentication, report storage, GPS-coordinate-based GIS mapping, real-time notifications through the Firestore Snapshot Listener, a KPI dashboard with Resolution Rate, and a points-based leaderboard mechanism. The system successfully digitized the OHS reporting process comprehensively and supports real-time monitoring of hazardous conditions in the port environment.
