Natural antioxidants are necessary to protect the lips from the drying out, chapping, and accelerated ageing caused by free radicals caused by UV radiation. One possible antioxidant found in starfruit leaves (Averrhoa bilimbi L.) is a flavonoid. The objective of this research is to synthesise lip serum formulations using an ethanol extract of starfruit leaves, then measure the antioxidant activity and total flavonoid content. A 70% ethanol solvent was used for the extraction process, which is known as maceration. There were three different concentrations of extract used to make the lip serum namely, 0,1%, 0,2%, and 0,3%. Methods such as organoleptic evaluation, homogeneity, pH, viscosity, and irritation were used to assess the preparations. The antioxidant activity was evaluated using the DPPH technique, and the total flavonoid content was determined using the AlCl3 colorimetric method with UV-Vis spectrophotometry. The findings demonstrated that every recipe was free of irritating ingredients and fulfilled the physical quality standards of the medicines. Formulas I, II, and III each had a total flavonoid content of 4.287 ± 0.040 mgQE/g, 4.408 ± 0.033 mgQE/g, and 4.497 ± 0.029 mgQE/g, respectively. Strong antioxidants were indicated by the IC50 values of Formula I (64.819 ± 0.363 ppm), Formula II (62.162 ± 0.136 ppm), and Formula III (60.518 ± 0.254 ppm).
