Introduction: Hypogalactia is a condition in which breast milk production is insufficient to meet an infant's nutritional needs. It commonly occurs among primiparous postpartum mothers due to limited breastfeeding experience, anxiety, fatigue, and other factors affecting lactation. Obesity may also contribute to impaired milk production and ejection. Katuk leaves are widely used as a local herbal galactagogue to support breastfeeding. Objective: To describe midwifery care for a primiparous postpartum mother with hypogalactia and obesity through local herbal-based education using katuk leaves. Methods: This case report employed a case study approach using the SOAP method including the study design, data collection methods, and data analysis. Data were collected through interviews, physical examinations, observations, and documentation during the postpartum period. Results and Discussion: Assessment of Mrs. R, a 23-year-old primiparous mother (P10001) on the fourth postpartum day, revealed low breast milk production, maternal anxiety regarding milk adequacy, and a BMI of 34.72 kg/m² (obesity). Interventions included breastfeeding education, on-demand breastfeeding, oxytocin massage, breast acupressure, psychological support, nutritional counseling, and education on katuk leaf consumption. By the 25th postpartum day, breast milk production improved, maternal confidence increased, and the infant's weight rose from 2,900 g to 3,600 g. Conclusion: Comprehensive midwifery care combined with local herbal-based education using katuk leaves helped improve breast milk production and support successful breastfeeding in a primiparous postpartum mother with hypogalactia and obesity.
